Scott F The degree to which a particular health insurance policy is restrictive about who you can see varies a lot - it has nothing to do with the health insurance marketplace. I get my insurance through the marketplace, but I chose a Blue Cross plan with a broad, national network (that’s more expensive, unfortunately). CTBarrister Jacob, I have fought this same issue previously over another Pouch expert, and I find it odd that they have put the burden on you and your GI to determine that "there is no one in network and in my city qualified to deal with my chronic issues related to my J-Pouch", when they should have the burden of stating who in network has such experience. mamaswans I take VSL #3 on a prn basis. My gastroenterologist frequently has samples that he generously gives me when i need them. It is terribly expensive, but is the best solution in my experience to calm a cranky j-pouch and get it back to "normal". Don't be afraid to ask for samples! [ more ]

Scott F Experimentation with fiber is very useful for folks to find out what works best for them. Psyllium is actually 70% soluble fiber and 30% insoluble fiber. Former Member I tried All-bran cereal from Kellogg’s a few days ago, and it dropped my frequency significantly. I ate a 1.5 cup bowl with milk, and I heated the cereal in the microwave for a minute, as I usually do with harder cereals. However, I read here on this forum that all-bran gave some pouchers terrible obstruction! So be careful! Number of toilet trips
ytcrockpot Totally agree with Scott, 4-6x a day for a well pouch, lucky if you go less. In my younger days I went about 2-4x a day with no meds, that was over 30yrs ago lol. finding a gastro that specializes in the pouch is a big help, but hard to find. I would travel the country (which I did) if it meant finding the right help for my pouch. [ more ]
lclassen I'm about 10+. Let's put it this way... I never go to the bathroom without pooping! My GI says that I have had a "less than desirable" outcome to my surgery. I don't have chronic pouchitis or cuffitis. Just had my pouch-oscopy this month and everything looked great! Don't need to have another one for 3 years! [ more ]
AWalkingTestimony On average for myself I usually go about 5 to 8 times on a bad day and 3 to 5 on good days. Like @Scott F I also take Psyllium fiber capsules and Imodium(only when I need to) to slow things down. After my surgery I was going around 10 to 15 times a day but things got better over time. I have only had my JPouch for a year and 2 months. Never had accidents. [ more ]

Former Member Did you brush with a hard tooth brush back then or did you brush your teeth over 2 minutes??? A soft toothbrush is always recommended over hard toothbrush. Brush time is usually recommended at 2 minutes. I am sorry to hear all of that as well! I could not imagine paying for all of that! Would you like to get dentures instead??? [ more ]
Pouchomarx I have bad gum recession. I do not have gingivitis or periodontal disease. Periodontist said usually with as much gum recession most have gingivitis or periodontal disease but I have neither. He said the recession could be from aggressive brushing over the years or could be hereditary. I know my dad had full dentures by age of 40 so I am guessing its that. Former Member Sorry you are in pain!! Hope you get better! Yeah, I agree with you 100%. A lot of people have had problems with certain exercises on here, especially involving core. I hope everything gets better dear. I think you maybe need to get checked out by your gastro. I would make an appointment, especially if you do not feel better soon. Please keep me updated I personally do not do much core exercises anymore. I only do about 10 situps and then I stop. That seems to not cause me anymore problems... [ more ]
KB1804 No, this was caused by some light core exercises. I’ve had no pain/gas issues recently other than this. I am still in a lot of pain, with cramping and reflux and I have not eaten at all today. [ more ]
Former Member I have experienced that before around my scars area before. I believe mine was pressure pain from gas and overeating. Whenever I over ate, I would feel that pain. I increased my water intake and ate little less and everything went fine. Are you overeating? Are you drinking enough water? And are you eating gassy foods??? [ more ]

BridgetB UPDATE: Thank you to everyone for replying. I finally got a dermatological diagnosis and it is, in fact, a form of eczema (caused by friction and moisture due to mask-wearing). I saw 2 other clinic doctors prior to the dermatologist and perioral dermatitis was on the differential, but it turns out, it's not that, and 2 days on the correct topical treatment has pretty much cleared it up - there's some discoloration but that will fade over time. tf Hi, I have been dx with siob & was put on Rifaximin: is a broad-spectrum nonabsorbed rifamycin antibiotic with an excellent safety profile, a lack of drug interactions, and minimal effect on the intestinal microbiome. Former Member I have experience with natural probiotics, I am not sure if that counts for an answer but I hope it does. I eat Oatmeal for a prebiotic and Greek Yogurt for a probiotic, I never had pouchitits either. Some people on here take Align, you can get it at a grocery story, I see it at Target all the time. Sometimes its even on sale for a MAD good price! I hear Kefir drinks are good for you too [ more ]

Pouchitis and/or Cuffitis
SteveG Might your pain be related to an anal fissure? I had one several years ago, it causes pain after going to the toilet if something gets stuck in the fissure. I thought it was pouchitis / cuffitis for several weeks, until I finally found out. With a fissure it helps to clean the anal region with water (shower), not only toilet paper. Pushing is usually not a good idea. If you are constipated, have you tried psyllium husk added to a meal with enough water? [ more ]
Boda My daughter had similar symptoms and it was cuffitis with a stricture. It was difficult to go and felt like a pushing a bowling ball out. It was due to inflammation. They did a scope and found it. They stretched it and used some sort of shot on it and it hasn’t come back. [ more ]
MJC I never have a fever with pouchitis and have had the same symptoms you are experiencing. Do you still see your Colo-Rectal surgeon or just your Gastro? I would call and get in sooner than 2 weeks! Good Luck. [ more ]

Jan Dollar Remicade has a similar adverse effect profile as Humira or any other biologic, so if those adverse effects are the primary worry, switching to another biologic will not be a solution. ytcrockpot @Minnesota Nice one of my doctors is similar to a functional medicine doc. He thinks outside the box, when I think most regular doctors don’t. Diet is a very big thing when it comes to Jpouches although a regular doctor just doesn’t go down that road, they usually send you to a nutritionist, I don’t find nutritionists help at all, I no what I can & can’t eat 40yrs into this and now the only med left for me is a biologic, it’s not something I’m looking forward to, but do hear they help. 8 week update. First day back in the gym

Jaydog It's been great. I've always liked lifting. It feels great to be back. Whether you join a gym or do resistance training I would say if you feel good then do it. Working out is great for your body and mind. I work out 4 days a week for about an hour or hour and a half per workout. For me that's about the only time I'm truly alone. No phone. I'm off the grid just listening to headphones and it's great. [ more ]

SteveG I have some natural yoghurt without any additives every morning, with fresh fruit slices, ground linseed and some nuts. The yoghurt you buy contains still some natural sugar (lactose). (If you produce your own SCD style yoghurt with a yoghurt maker you can see the difference, that tastes really sour.) But luckily I can tolerate lactose in small amounts. Scott F If you don't go beyond the insurance company-managed appeals then you're unlikely to succeed. Ultimately I caused CareFirst BlueCross to develop a fairly liberal policy wrt VSL #3 DS. The key language is: "VSL#3-DS®, available by prescription only as a medical food, may be considered medically necessary for the dietary management of an ileal pouch and ulcerative colitis. Scott F Pouches can definitely be made again, depending on the cause of the first pouch’s failure. It sounds, though, like you might just have a simple stricture. These can be a significant nuisance, and sometimes repeated dilations are needed, but they don’t generally require a pouch re-do. [ more ]

Cipro vs ?
Former Member Yeah Cipro has a tendency of thickening up stool. I Definitely increase water intake as much as possible. I think that can possibly help your situation, also moving around as much as possible. [ more ]
Scott F If the Cipro (and rifaximin) are just making the stool too thick to pass comfortably, perhaps they would work acceptably if you took some steps in advance and during treatment to offset that effect. Some possibilities include 1) drinking lots more water, and maybe adding grape juice, 2) stopping all bowel slowers (Lomotil and Imodium) if you take them, 3) tinkering with fiber dose (it’s often recommended for constipation) or stopping it, 4) adjusting your diet to reduce/avoid binding foods. CTBarrister Tinidazole was a core antibiotic in my rotation. However unlike you I tolerated all of them well and they all worked well on symptoms before I finally went on Remicade and put antibiotics in my rear view mirror. I believe tinidazole is in the same antibiotic family as flagyl aka metronidazole. BTW Bentyl is an anti-spasmodic, not an antibiotic. [ more ]

JHendrix I didn't end up speaking with the doctor as the discharge disappeared as quickly as it began. It was very unusual but has returned to 'normal'. Who knows. I panicked prematurely...not the 1st time! [ more ]

Scar Tissue and Chronic Obstructions

Rachel95 Thanks Lauren!! I used to be able to prevent obstructions that way but it seems that I’m chronically partially obstructed at all times these days no matter what (getting enough water always helps tho!). I’d love to know if you or anyone has tried therapies alternative to surgery to help break up scar tissue? I can’t seem to get rid of this partial obstruction (going on two years with it and it’s miserable haha). [ more ]

How diagnose Crohn's for sure?
Former Member Maybe he should keep the ostomy for now until everything is factual. I will agree with the pill test. I do trust the Prometheus kit a lot. I had both tests done and that came back Ulcerative Colitis for both. Maybe he should get both done and see how the results come back. Hope this helps. Scott F Crohn’s can sometimes be diagnosed with certainty, but it’s often hard to be sure. The pathologist is the most reliable source of information here, but I can’t tell from your post how confident the pathologist was in the Crohn’s diagnosis. You could ask for a second pathological opinion from a GI pathologist (the subspecialty is called “Gastrointestinal Pathology”). You may get some push-back, since second opinions in pathology are uncommon. Many (perhaps most) surgeons in the US won’t do... [ more ]
CTBarrister He could have another Doctor view/analyze the films. Yes, I have had my J Pouch for close to 29 years. What they are calling Crohn's developed 15 years after, whether that is due to backsplash stool from the J Pouch or a self contained inflammation pattern within the ileum has never really been established and probably does not matter. Waiting on MRI results, I feel completely fine now after what I thought was a really bad hemorrhoid so this is all coming as a shock. Jaypea I have had several abscesses following my j-pouch. Please make sure you see a colo-rectal surgeon if you are going to have it surgically managed. I had one abscess turn into a fistula but I am completely healed now and have not had an abscess for over a year. [ more ]

Former Member Convatec sensi cream helped but did not take it away, it made it tolerant, that was my best friend at the time!!!! You will probably get use to everything over time. I found that drinking salt water helped my butt burn tremendously. AND WIPES!!!! [ more ]

Melissa (MCJ) Hi, I'm so sorry I didn't get back to you sooner. Thanks so much for all your tips. I do some of them, but icing before and after is a good suggestion and massaging afterwards. Also have around 7-9 bms in 24 hours and no leaking at night anymore, so that's great. Be well. [ more ]
pouchmonkey Hi Melissa! I also have the red, itchy injection sites. I’ve been on humira for a year & here are a few tricks that help-let the pen sit out for 15-30 mins to warm up so the medicine isn’t like thick gel or it is definitely painful. You can I’ve the skin before & after the injection, which also helps. Make sure to move the site around, switch legs & areas, & pinch the skin up with one hand, inject with the other. Also, after the injection, take a towel & massage the area. [ more ]

Pelvic floor disorder (uterine prolapse) - caused by j-pouch surgery?
genreina Hi! I'm so sorry you're dealing with this. I ended up having several appointments with physical therapists, which did help (insurance was a problem, but eventually got that taken care of). Part of my problem seemed to be that I had virtually no core strength, which also led to lower back issues. But also, the article linked in @duck11 's post was very helpful, both for me and the physical therapist, to understanding what was happening. Scott F When you tried Metamucil did it really have no effect on your stool consistency? What dose were you using? I’m starting with Metamucil because it works in a very simple, mechanical way. tulsamom I wish I could take them again. You all are reminding me of why I try them again every year. It’s about that time again. 😂 in the olden days, they did make a difference. But each one pooped out after a while and I’d have to switch. That happens with some people. Since 2013 I haven’t been able to take them because of severe side effects. X-rays in underwear
Former Member I think each situation might be different. I have taken many X-rays while my underwear was on and some with a hospital gown, some even with bra on I believe. II have never been naked though, that would be creepy lol. I believe I have had doctors position me as well when it came to radiology work. I know things can be highly uncomfortable but I would just get everything done to make sure everything is okay. Of course if there is something skecthy going on, definitely speak up and use your... CTBarrister Jaypea, I understand you are in the healthcare profession but I am 57 years old, have been a "professional patient" most of my life, and never had to have a "chaperone" in the room until the episode I described above. It's very simple: if a doctor is telling me he or she doesn't trust me as a patient, or else can't trust himself or herself, I do not want to be their patient. I do have the choice of going elsewhere, and I exercised that choice. [ more ]
Jaypea X-rays without a simple gown and only briefs is unacceptable. Had she needed to position you more correctly she could have asked you to raise the gown above your hips. Having a chaperone is quite common. As a nurse in a pediatric ER I was often asked to accompany a male physician into the cubicle when he was examining a female patient on her own. [ more ]

Scott F There are medications that can help resolve anal fissures (nitroglycerin, diltiazem, verapamil). Sitz baths can help. Keeping stool from getting too firm can help. Definitely ask your doctor. If what you have is external then it wouldn’t be an anal fissure, but your doctor might still be able to help. [ more ]

Covid questions
Lisa H I am recently recovered from Covid. I don’t think I’ve been that sick in a very long time. Difficulty breathing, constant dry cough, fever, aches, migraine-like headache. I was fine for the first nine days and then almost overnight the symptoms appeared. I went to an urgent care center where they’ve been treating hundreds of patients. tf Hi, I was dx with UC in 98 very bad case and after take down and Jpouch in 2013 I still have UC, IBD. My immune system is not 100% and I don't take drugs except for Lomotil prn and Hyoscyamine prn. My primary Dr who I just seen said that I should put on form immunocompromised because I am. I take my vitamins and exercise and do I need to stay strong. The fact is I still am not because I lack nutrient and the more u put in the more comes out no matter what for me anyway.
