Jan Dollar I have never heard of IgG for gut health, so I had to look it up. Usually, IgG is given IV or subcutaneously. But, in this instance, it is an oral supplement. Even though it has been around for years, it does not seem that there are clear indications of efficacy. I am not sure how it survives digestion. Like probiotics, perhaps it is a mixed bag. Update - New GI
New577 Hi, IMHO 6-9 is not high frequency. I was quoted by several surgeons prior to choosing one, that I could expect 6-8 BMs per day. And that’s exactly where I am at 3.5 years later. like you, on bad days; IPS, cuffitis, or SIBO flareups the number goes higher. on days where I only have 4-5, i know the following days I will have more as the small intestine needs to clear itself out. please don’t stress, your outcome is your outcome (same with me). Scott F That's useful to know, I think, even though there's not much you can reasonably do about J-pouch size, short of surgery. It's possible that your surgeon chose to make the pouch smaller in order to enable the blood supply to reach properly (I'm not certain that the anatomy works that way, but I think it might). It's also possible that your surgeon simply didn't do an ideal job. I think your GI's advice about managing the inflammation is exactly right. [ more ]

Iron infusion

Nancydiane When you say it doesn't like them did it cause diarrhea or do they just absorb better sublingual. Hoping this is a one time deal as I'm still not tolerating a lot of foods yet which was thinking could be why not getting the iron needed but I don't know I'm very new at this. Thanks [ more ]
New577 I have iron infusions once a year as my hemoglobin drops too low. iron supplements I cannot tolerate and it’s just easier to have the infusions and be done with it for 12-15 months. unfortunately us pouchers have malabsorption of vitamins and minerals that are processed in the terminal ileum. I have to take vitamins d and b12 sublingual as the pouch does not like those either. [ more ]

AlabamaMike Dr. Stephen Clark in Huntsville has been doing my pouch scopes and he repaired an umbilical hernia. Great Surgeon. He is with the Center for Colon and rectal surgery on Manning Drive in Huntsville. Dr. Bruce Belin in Lexington, Kentucky built my pouch in 2004 and I would highly recommend him. [ more ]
Dude, where's my colon? I had my surgeries with Dr. Campbell at Huntsville Hospital in Alabama and I had a good result. During my early UC days I was treated at both UAB and Vanderbilt. I had a much better experience at Vanderbilt. The GI team up there is great. [ more ]

Looking for central Massachusetts GI
Nikino A week to reply, that’s not good. A patient portal is so good for a doctor’s office to have. My prior primary care doc in Worcester did not have one. Makes things difficult. I don’t know any GI docs in central MA current day, I used to go to Elise Jacques, MD but she retired. I’d think most docs have some type of portal now maybe UMass hospital has a good GI doc & portal. Not sure if that’s closer for you. [ more ]
roxz Thanks! good to know their good. I’m over an hour from Boston, or would still be there went to the Chrons and colitis center affiliated with Massgeneral. They were great but the drive not so much had to take a whole day off work just to go to appointments. The worst thing about my doctor is they don’t have a patient portal to communicate always have to call and take up to a week to hear back typically. [ more ]
Nikino If you are in Boston at Mass General Brigham Hospital, MGH- in my opinion, I feel safe to say you’re in a good spot. Communication is key. Keep messaging through the portal with when needed. [ more ]

Scott F Tylenol is very safe in the usual doses (e.g. extra strength). The higher doses can be risky if you drink alcohol or have liver disease (whether you know it or not). [ more ]

Jan Dollar I have been on Remicade since 2016 (on biosimilar Inflectra since 2018). I would not second guess your choices. Skyrizi was not even available when I started Remicade and I had failed other biologics. There just is more data with Remicade because it has been around so long. So, I may wind up on Skyrizi at some point. I just started with a new GI and we talked about the newer biologics. But, he’s of the notion of sticking with what works. He even said he is not inclined to mess with my meds... [ more ]
ytcrockpot Hi Jan, May I ask how long you’ve been on remicade? My doctor gave me choice between remicade & Skyrizi. I’ve seen a few posts stating remicade is working very well for inflammation & pouchitis. chili123 So, if I have chronic inflammation in the terminal ileum, how can they tell if I have more issues farther up? My gi doctor, who is new for me and also new in her practice, had a lot of blood tests done and stool sample.....and all were pretty good...2 were just over normal.....stool was 56 and normal was up to 50....so I was sure that my biopsies would be okay. Fiber on the go
Jan Dollar How about Benefiber? Mixes clear and you can put it in water or anything. No taste. No grit. Metamucil always gave me a ton of gas and diarrhea. I guess I am sensitive to it. [ more ]

Len78 hi Jan, Diarrhea as a day to day and more frequent trips than just what a jpouch brings wo gastrectomy are some of my fears uncertsin transit time and winding up needing pull ups due to dumping syndrome comming on are also fears no one has answers, surgeon doesnt know how it will affect me post colectomy and living with jjpouch not having a stomach I want to be able to go shopping for clothing and food on my own after recovery which takes 6 months to a year ill have b12 issues and require... Jan Dollar Artificial sphincters have been around for at least a couple of decades, mostly for urinary incontinence. My dad had one for urinary incontinence following prostate surgery. It was successful for him. But, urinary incontinence is different than fecal incontinence. There are a litany of complications, but the main one is erosion through the internal walls. Jan Dollar I always do clear liquids and a bottle of mag citrate the day before. My new GI ordered the standard sigmoid prep with clear liquids, Miralax, and mag citrate but that is overkill. I guess it all depends on your own transit times. Jan [ more ]

Gastros instructions for first flex sig since transanal mucosectomy was done 4/17/24
Jan Dollar The reason you don’t need the saline (Fleet) enema is because it is hypertonic (concentrated) to pull water from the colon to stimulate evacuation. We do not need that, just the rinsing. It is the container that we need, and rinse till clear. It does not stay in long enough to really matter one way or the other. I have been doing the mag citrate, but it does not bother me, and I do actually wind up expelling some retained solids that way. Jan [ more ]

SteveG The first 3 applications should be infusions at the doctor's, with a higher dosage (600 mg) than the injector, which is used afterwards in 8 week intervals (at home). [ more ]

Still Standing I'm currently on Skyrizzi and have been since March of this year. I don't have any major side effects that I'm aware of. We'll know more how it is working when they scope after a year from when I started it. I also take Budesonide and Imuran beside the Skyrizzi. The on body injector is different from just getting a shot, but you'll get the hang of it pretty quick. (It takes 5 minutes to inject the medicine.) I wish there was a magic formula that fixed us all without chemicals and side... [ more ]

There was a court action lodged against this company alleging inflated claims for its particular version of this product, and I would hate anyone to click through, believe the hype, and spend a lot of money on this because of a thread that I started. Superstarch is a legitimate product in its own right (not "just cornflour") but it appears that this company may have exaggerated its benefits to athletes for sales... [ more ]

Anal Stricture - Surgeon recommending dilation ...then what?
Hazel Hi! Im from Maine also and had Dr. Schoetz, the worlds best colon rectal surgeon! I have a flex/sig scheduled later this month with Dr. Marc Branda. He’s very good. I’ve been seeing him locally for a while now - got a little tired of driving to the Lahey Clinic, although I do miss seeing the doctors there. Len78 Hi Scott, thank you very much . So with my pouch having been moved she hand sewn I have a new anastomosis being the atz was removed with the associated length 5 cm cuff. I’ve always had polyps in my jpouch but I’m sure if surgeon felt negative about saving my pouch and the mucosectomy itself he wouldn’t have persued it in the 1st place i don’t at an ostomy there’s no more tinkering with the pouch after this but the procedure first take away te jpouch polyps what I had done thanks and I think... Scott F I’m pretty confident that your procedure would have eliminated any polyps at the old anastomosis. Your new anastomosis should be less prone to polyp formation. You can confirm this in a couple of weeks when you go for your pouchoscopy. [ more ]

Ljz Yes!! Thanks for sharing that! I will definitely call- and my husband will call and I will call again to request it! I did before but never got through nor did they answer emails. Would be wonderful if Ilex became available again! I too have found Kens compound to be similar to Ilex but because the stomadhesive burns I was using maybe 20% of that with 80% of the Cerave which is nice and thick. I will try the alchohol free convatec paste right away but very thankful to ken for finding that... [ more ]

I called them, and while they suggested trying "Remedy," I found it doesn’t compare to iLex in terms of consistency or effectiveness. The representative told me she will let management know. tulsamom Dr Remzi saw the cause of the jpouch growing up in the abdomen as a restriction near the bottom. So first surgery to create ostomy and let pouch and surrounding tissues heal and rest for 6 months. Then at 6 months- surgery number 2 - he will remove the restriction in the mesorectum and will either reduce jpouch to normal size or create a new one. At 9 months from first surgery, Oct 24, he will hook the restored jpouch back up and close ostomy. Much more extensive and disturbing situation... [ more ]

How to settle down diarrhoea
CTBarrister All Pepto Bismol is, is bismuth subsalicylate. Its a metal which is toxic to bacteria in your bowel. It is also a mild antacid so I would imagine it is at least somewhat alkaline. I seriously doubt it will cause any burning sensation on the way out. I never experienced that. [ more ]

Which reminds me that medical offices have become huge recent targets for ransomware attacks. I work as an attorney and handle many personal injury cases, and just in the past year, settlement in a couple of my cases was held up due to such attacks. Former Member @JDsLightLab Have you ever tried a cheaper version of wipes??? The wipes you use are pretty expensive, just wanted to make a suggestion for you to save money. I use fragrance free wipes from Target (target brand) and target brand also makes sensitive skin moisturizing wipes as well, if you do not have a target in your area- i am sure walmart may be in your area. Have you tried or thought about using cheaper brand wipes??? [ more ]

Scott F First, be patient with yourself. You’re recovering from surgery, and your body will keep changing/healing for a while. I take one full dose of psyllium (per the package directions) before breakfast and dinner. It’s easy to experiment with the quantity to see what works best for you. I actually mix a half dose of Metamucil and a half dose of Konsyl, but the amount of psyllium is the same. I take one Lomotil at bedtime to help me sleep through the night. I rarely take a bowel slower otherwise,... [ more ]

Blood test
Jaypea CRP is merely a marker for inflammation. It could be high for any number of reasons. I once had a CRP of over 300 and the doctors never did find out why. I was fine two weeks later! [ more ]
Scott F The Prometheus test is the only blood test that purports to distinguish between Crohn’s and UC. It’s not very reliable. The distinction often isn’t very important - in my case they never actually figured it out. If you have active inflammation it should be treated, of course. CRP can help assess inflammation, and there are a variety of other tests that also can help with that. Unfortunately they can disagree. [ more ]

Good days and bad days
CTBarrister Hope, I am doing good and have been for a long time. If you had surgery in 2022 you could be experiencing a lingering motility issue. Try taking the lomotil as I suggested 45 minutes before meals and if you see a noticeable decrease in BMs right after eating meals, it's definitely a motility issue. Lomotil is the drug prescribed for such issues, as suggested by its name although a better name would be Slomotil. As in slower motility. Many doctors call this phenomenon "spasmodic pouch",... Hope Its OK Thank you for the detailed reply. My doctor did not mention that about Lomotil. He only said to me to take it morning-noon-night twice each time. But to be honest with you, I feel like it does not help sometimes. I’ll try your method for a few days and see if it helps. About motility, I’ll also research that. I’ve had my latest operation in 2022 summer so it’s been now 2 years. My doctor keeps giving me this iron pill that makes me ill with so painful diarrhoea and does not plan to give me... [ more ]
