I had j pouch surgery in 2001 and fistula repair in 2015, since fistula repair my incontinence issues are much worse and I’ve had many occasions I’ve had major stool leakage at night, sometimes several times nightly…very frustrating! Since fistula repair I’ve also developed chronic pouchitis and this causes me to have daytime incontinence as well. I also have sensation of not being able to empty my bowel completely, feeling of pressure and spasms. I take Cipro and Flagyl and Dr. Martin brand of probiotics and Imodium 2 tablets every morning. Since fistula repair my j pouch performance has deteriorated big time. Anyone out there with similar issues and any advice would be appreciated. Thank you.

Do you think the antibiotics are controlling the pouchitis adequately? If antibiotics failed for me I’d start trying biologics. I have no reason to think they’d do much for the fistula repair complication, though, but they sometimes do great on pouchitis.

If you can get the daytime incontinence back under control, maybe the Imodium would be more useful at bedtime. That’s when I take Lomotil, for improved overnight security.

Oil of oregano is not of any interest to me. I am much more cautious with “supplements” than many folks. I do use potent (but pretty well tested) probiotics (VSL #3, changing soon to Visbiome), and they also might conceivably help you, at least by making your wallet lighter to carry around. Another long shot to consider would be pelvic floor PT.
