When you are at a new doctor, etc. and the form says do you have any medical conditions we need to be aware of?

What do you write?

I feel that although I no longer have UC, I still have occassional medical issues related to a J-pouch. We technically do not have a disease, but our bowels do sometimes act up (pouchitis, blockage, etc).

I am thinking that I should just write something like this.

"Potential complications from J-Pouch surgery to remove the colon due to Colitis."

I write: K pouch (continent ileal inter-abdominal reservoir)following a total colectomy due to bowel disease and (in my case) congenital deformation. Potential complications include: occlusion, ileus, pouchitis, adhesions, non absorption of coated LTP and coated meds and other relative complications due to an abdominal pouch.
That is usually sufficient to give the doc a basic on what I've got.

I usually write: ileo-anal anastamosis (j-pouch) secondary to Ulcerative Colitis. Then I tick all the boxes (you know the ones that say "do you currently have or have ever had...) about blood transfusions, anemia, surgeries, steroids, arrhythmia, hospitalizations, etc, etc, etc, and then I usually put a big bracket saying "re: Ulcerative colitis." Depending on the specific form, sometimes I tick so many boxes, I can only laugh about it.
