Hello,

So its been a while since Ive posted. I had both of my surgeries in 2007. Every year I am told I have a "healthy pouch" by Dr. Shen.

So for the past year or so, I have had VERY loud stomach gurgling. Mostly in the evening. A lot of gas when I go to the bathroom. I never thought anything of it....until recently when I came on here and discovered it could be SIBO. So i diagnosed myself with this. The funny thing is that I recently was doing a low carb diet to try to lose weight and the gurgling almost completely stopped. I stopped doing the low carb because all the veggies were so hard on my pouch too.

So my question is...what diet do you guys find is most helpful with SIBO? SCD??

SCD, Paleo, FODMAP, UMASS, Atkins, all are similar in some respects- they all reduce consumptions of carbs and sugars and increase consumption of proteins and fats in lieu thereof.  Emphasis of Paleo is processed food elimination.  Emphasis of FODMAP is elimination of certain carbs which are viewed as bad.  I have studied the UMASS IBD diet and can't figure it out quite honestly, although it has attributes of the other diets.

My conclusion, having tried all of them (but mostly Paleo as I believe in its tenets and philosophies and, quite frankly, its disciples have argued a bit more logically in my mind than the disciples of the other diets) is that there is no right or wrong diet.  You must experiment and conclude, experiment and conclude, and then experiment and conclude some more.

Go on each of the aforesaid diets for one month, report back the results, and let's discuss.  But please note the battle against SIBO is more like a 1 year battle than a one month battle.  It could take months before you see results on any one diet.  This is a caveat.  So you could try a year on each and then report, and if you do this we can talk about writing a book on the experiment maybe.

WOW! thank you for your response. I have never heard of the UMASS IBD diet (but I like the name ) I think I am going to look into doing the SCD diet first. I would like this to become a lifestyle change. I know sugar is so bad for everyone, esp us pouchers so I would like to keep it out of my diet for the long term.

You are such an inspiration with having your pouch for 43 years! Good for you!

The UMass diet is derived from the SCD and is the only diet purporting to be specifically designed for those with IBD, although the Paleo Diet has spun off at least one book for those with IBD.  More on the UMass IBD diet here:

If you don't have the time to leaf through all the recipes in the second link, I actually did.  The diet makes no sense to me, applying what I know about nutrition and what I know about IBD.  I assume there is some science behind it and that UMASS is not employing knuckleheads, but you have to know that I am a graduate of UMass's rival UConn, so it is difficult for me to make such assumptions.  LOL.

You really should take the time to educate yourself on this subject.  I have tried to.  I have spoken to multiple registered dieticians (one of whom is a client of mine) and medical doctors, in unison.  I have read a few books.  The books tend to have more subjective opinions than empirical, research-based science.  Which is why I stopped reading them.  But there is some science out there, you just have to look for it.

By the way I have had the J Pouch for 23 years and IBD for 43 years.  When I was 9 years old they didn't have J Pouches, but they did when I was 29 and needed surgery.  When I was 9 years old, they did not have much to treat UC and they did not have sedation of any kind for colonscopies.  I have lived with UC and now Pouchitis/SIBO for the majority of my life, which is soon to be 52 years old.
