Very sore, red raw skin around loop ileostomy Stoma.....help !

Recently had J pouch created and have a sunken loop ileostomy.

Been advised by Stoma nurse to use a convex bag or wafer/flange together with a convex barrier filler ring.

The rings/wafer etc keeps leaking and my skin around the Stoma is red raw and extremely painful.

Both the Stoma nurse at the hospital and my local Stoma nurse advise against a barrier cream or a spray such as Cavilon ?

My local Stoma nurse recommends I use a fine sprinkling of OstoSeal protective powder prior to applying the bag rings/wafer, which I've been applying for at least a week and without improvment.

I believe this sprinkling of powder is making matters worse by preventing the bag/wafer etc from adhering to my abdomen.

Any suggestions, as I'm in so much pain and discomfort.

Thanks Jan, I'll bear that in mind.

I feel that if I wasnt advised not to use the Cavilon, I wouldn't be experiencing what I am now.

After the loop ileostomy was formed, I was using the Cavilon for at least 10 days whilst still in hospital and without issues, the red raw skin appeared within days of following the advice of my Stoma nurse.

Can I apply the Cavilon to red raw and sore skin ?

According to the manufacturer's instructions, it can be applied to damaged skin. Whether it will help or not probably depends on how excoriated the skin is. You can even use the powder and apply the barrier spray on top. Making sure it is dry before applying the flange.

Many people here swear by Eakins seals around retracted stomas to improve the seal of a convex system. Is that the brand of seal you are using? An ostomy belt may also help maintain the correct pressure to keep a good seal. Once the seal is good, then the healing can begin. Hopefully others with personal experience will respond.

I have the same issue strange. I had my surgery on 12Th may

Currently using a 2 piece mouldable convatec bag with convex.

It's working better than any other bag I've tried but still get skin irritation and bleeding. I use the powder and spray. Yesterday I rubbed the powder into the wound, I'm curious to see if that's made a difference when I next change the bag

No, never used or needed a seal and intially my Stoma was very short and close to the skin, on occassion a it would completely retract.

The flange would and could stay on for at least five days maybe six but due to bathing I would change it every 3 days and for my own piece of mind, but this wasnt essential.

Due to the output of my end ileostomy being very thick, like stodgy porridge, I ended up using closed bags which I prefer. I would change the closed bag three, four or five times a day. I've been advised I won't be be able to use closed bags as the output of the loop ileostomy is very watery and like liquid but at present and on most occasions, the output is like the consistency of thick custard, I'm experience the same issues when emptying the bag as I did when I had the end ileostomy ?

I was doing the same but the powder seemed to prevent the flange from sticking to my skin and appeared to do nothing to relieve pain or prevent further skin damage.

Since I've used the Cavilon, although still stinging, it's slightly more bearable but at least the bag, upon till now hasn't leaked.

I'll try the powder followed by the the Cavilon and see what results that achieves, however, the cream suggested by Jan

Update:

ilex Protectorant appears to be available from UK NHS.

I've ignored my Stoma Nurse suggestion; applied only Cavilon spray barrier solution to the angry and inflamed skin around my Stoma and reverted to using the ConvaTec Esteem Synergy 2 piece appliance with a filler ring.

Since applying the Cavilon, the skin around my Stoma, although still red and sore, is no longer painful or uncomfortable and is looking much better than the oozing, festering wound it was.

Update.... Especially for forum members within the UK.

ilex Protectorant is NOT actually available on NHS prescription.

Some hospitals will supply it, I guess only samples sachets via the Stoma Nurse.

As that's what I've finally received from my Stoma Nurse.

Yeah Shainy, when I began using ConvaTec pouches all those years ago I experienced the filters leaking which stained many of my shirts.

Since the shirt stains, I've been using ConvaTec bags/pouches without filters.

ConvaTec offer a drainable pouch without a filter too.

When using the stickers, it's best stick one over the filter from the inside as well as the outside.

The ilex protectorant I refer to; is a paste which when applied after the Cavilon Barrier Spray protects the skin around the stoma from being eroded away by bile acid.

It can be applied directly to the skin around the Stoma or applied to the back of the flange/wafer/collar or whatever is applied to the skin first, the latter, I find more effective.
