Antibiotics is the first line of treatment and biologics are the second line of treatment. Chronicity and progress of the disease in the face of treatment will determine the course of treatment over time.

You don't mention if "J Pouch passage" means the J pouch inlet or outlet. I have had inflammation at the inlet due to backsplash stool for many years and it's very common once a Pouch gets to be around 15 years old. Over time inflammation can cause the stricturing of the inlet or outlet and then it will at some point need to be dilated. I did have to get dilated in 2021 from 7 mm to 18 mm at the inlet.

These are "been there and done that" situations for many people with 25 year pouches. From my own experiences they are not really as big a deal as they sound.
