The stoma and abdomen are usually too tender for several months after the surgery to lay flat, but after that he would most likely be able to. People usually talk about not being able to SLEEP on your stomach with an ostomy, which is because your bodyweight prevents the bag from filling. If it's just a few minutes to paddle out I think it would be fine.

The real problem would be protecting the stoma from trauma during surfing, but there are lots of stoma guard products on the market for that.
I agree with Pluot. Your son may want to take up Stand-up Paddleboarding (SUP) instead. You're either on your knees or you're standing up and you can still catch waves just the same as surfing but not from a lying down position.

I've been surfing for roughly 30 years and have had an ostomy bag between take-down and reattachment. I can't really see how one could surf (comfortably) with a bag.
Having an ostomy can make things like this difficult. While I'm sure it's "possible" to surf with an ostomy, it is going to make things much less comfortable.

That said, if he is sick, he isn't going to be doing/enjoying surfing much anyways. At least with the ostomy he will feel better and be able to pursue an overall more active lifestyle, even if it doesn't include surfing. Unfortunately, sometimes we are forced to pick the better of two less-than-ideal options.
