Hello - I've had my j-pouch for 9 years and had 5-6 bouts of pouchitis, usually right before a trip. Over the years I've been prescribed cipro or flagyl; both have been effective, although the first time my (former) doctor left me on cipro a long time (years) and I got C-diff.  This time I'd like to try a probiotic, to see if I can prevent more pouchitis or another C-diff infection. Does anyone have a current recommendation? I found a lot of posts about probiotics on this site, but most are many years old.  I found this current meta-analysis article:

It shows that in small studies, VSL #3 is significantly effective in preventing pouchitis. However, VSL #3 is way too expensive for me; I read that Visbiome is similar, but it's also very expensive. Also, I'd really like a shelf-stable product so I can travel with it. I will appreciate any advice! Thank you!

I also have chronic pouchitis & cdiff. I’m allergic to flagyl & cipro.
I’ve been trying to find a probiotic that would help with the pouchitis, I’ve done the over-the-counter ones to no avail and spent way too much money trying them.

So decided to try VSL#3, I purchased it on Amazon during the prime day sale and got it for a much cheaper. And I must say it’s the best that I’ve tried. I only take 1/2 for right now, shake it on food, hoping it will continue to work long-term which will help the pouchitis and I can get off my steroid.

The probiotic that is supposed to help prevent C-diff is Florastor (or other S. Boulardi formulations). That’s a yeast strain, very different from bacterial probiotics. The study you read about “VSL #3” was actually studying the product now called Visbiome. The product formula had separate ownership from the product name, and they had a little tiff. Nevertheless, they are indeed very similar. I use Visbiome now. It’s expensive, but you might want to compare prices per CFU - lots of probiotics have lower prices because they include less active ingredient, and some of them don’t even reveal how much.

Hi, for C-Dif prevention, nothing beats saccharomyces boulardii. It is ridiculously effective.  I kept getting C-Dif  every time I was on antibiotics...until I started taking S. Boulardii during, and a couple months after, antibiotics. My GI doc now has every patient take it with their antibiotics and he has been very impressed with what he claims is a 80-90% reduction in C-Dif cases in his patients.
