Hello!

I recently had routine pouchoscopy (no issues I am aware of). Tonight I had the results of the scope and biopsies come through mychart, along with a note from the doctor that things look great and he would see me in a year. However the results don’t look ok to me. It says there was some inflammation at the inlet, and the biopsy of the cuff shows inflammation too, even though it looked ok on the scope. He says no treatment needed. Can someone reassure me these results are normal for us? I’m just such a worrier because my first two years with this pouch have been tough. I’m attaching the report. Thank you for taking the time!

In my non medical expertise opinion I think you are ok. I also have yearly scopes with similar results.

i too have chronic UC of the cuff. Because I had high grade dysplasia in the rectum, I still take one Canasa suppository daily, to keep the cuffitis in check and prevent a reoccurrence.

I also have a slight narrowing of the anastomosis, which the GI dilates  with his finger, and tells me it’s clinically insignificant.

The important question is how do you feel?

Pouchitis, Cuffitis, SIBO and Irritable Pouch can make you feel miserable. I have had the  last three above from time to time.

The GI after each scope tells me everything looks good, so I try not to live in the terrifying future.

if you are feeling ok, I would think you are doing well.

I was told by my doctor that microscopic inflammation is the norm and treatment is based on what things look like through the endoscope and clinical assessment.  In other words, some inflammation in the biopsy is to be expected and  if I don't have symptoms, probably nothing to worry about.  

I don't have a cuff, though.  Your report looked good but mentions chronic active disease in the cuff.  I always understood that it was important to treat active colitis (back when I had a colon.)  So perhaps that's a worthwhile topic to discuss with your doctor, if "chronic actively inflamed colitis" is what's showing.  The canasa/ mesalamine suppositories are the standard treatment for cuffitis.
