Hello,

 

I have had a jpouch for 9 years and recently had my first pouchoscopy.  The GI (not my surgeon) took three biopsies but he said nothing indicated any sign of cancer that he could see.  Are biopsies normal or should I be concerned? He also said there was some erosion around the surgical attachment site?  And he said he had some mild inflammation around the rectum and in my notes I noticed “proctitis.”

 

should I be worried?  I don’t have bad urgency or anything just has discomfort and water BMs but no bleeding.  I’m discouraged.

Biopsies are completely normal and in no way indicate a suspicion of cancer (cancer in J-pouches is rare). The rest of it is consistent with your discomfort and watery stool, and might enable him to suggest some treatments that might help you feel better. That’s the opposite of discouraging, or at least it would be for me.

So this was a routine pouchoscopy, with no symptoms causing you to get it?

The bloodwork is probably to help figure out what’s causing the erosion and proctitis. Whether the erosion is likely to get worse depends on what’s causing it.

All of us prefer to stay off of medication. It’s also reasonable to be wary of any specific medication that made you sick or failed to work in the past. It’s also sensible to be cautious about taking medication if you have no symptoms. It’s best, though, to think through any proposed treatment logically, and get any questions or concerns addressed, rather than assume that all medications will be just like one particular (and unhelpful) UC medication was.
