I have been having pain and urgency when I have a a BM the last few months. Each time I call my doc and he says it could be pouchitis or stricture. I have gone in a few times to see him and he say there is no structure and that I should just wait untill July when my 1st yearly scope is scheduled. He has put me on Cipro and Flagyl a few times and I feel great on those! A few days off of them and I am in pain and cannot empty completely. The pain also feels like swelling and irritation in the cuff not higher up in the pouch. I tried doing a gloved finger exam myself to see if it felt bumpy but I could not even begin to get a finger in due to pain and swollen area. I can't completely empty either and it feels like I need to go every 5 seconds. And I am getting sharp spasm like pain on the left side of anus. I mentioned maybe cuffitis to my surgeon and he just blew me off and said if that was it the antibiotics would not have made me feel better. I thought maybe fissure too. Occasionally there is a small bright red spot of blood on the toilet paper. Well that's a run down of symptoms any suggestions? I'm going to call and demand a scope in the next week or two. Oh and i also have a general sense of pelvic discomfort and lower back pain. My reversal was last July and up untill a few months ago I have felt great. I also feel tired all the time. If I'm laying down I feel ok.

Your symptoms are very similar to mine. I have chrinic cuffitis and pouchitis in my distal pouch due to an anastomotic structure. My pouch functions fine for the most part but my cuff and anal canal give me pain 24/7. May not be the case for you but a scope should reveal cause. Your emptying issues could be due to inflammation in the cuff and ATZ as I aski experience those symptoms. Unfortunately for me no meds are really working So I'm looking into pouch advancement after scope next month. Call your doc and get the scope before your annual.
