I have had a J since 1986, worked fairly well until 2001until a stricture revision,  which made me near normal until the past few months. Now I have a stricture at the inlet, the plasty didn't seem to help. I started having pain, pressure & dribbling recently, can't sleep. I was taking NSAIDS for pain, but they caused ulcers above the stricture. Last night I took a Lomotil, which has mild  narcotic effects. Killed the pain and  I slept like a baby. Great until the next day when the constipation was aggravated  by the Lomotil. Any alternatives?

Direct pain relief and narcosis from Lomotil is very, very close to zero. OTOH by slowing the bowel it would have reduced uncomfortable pressure on the stricture. You don't describe your stool consistency, but if you can find the thickness that most easily squeezes past the narrowing, then maintaining that is very valuable, particularly if you can maintain it along with a bowel slower. Ways to manage that consistency include water consumption, soluble fiber, and food choices. This is more of an art than a science. I hope you are able to get on top of this!

Thanks. The stool when I was good was somewhat solid, and gas would pass behind it and clear the pouch out for a few hours. I had a fairly normal life. Now since the stricture has gotten tighter I just have liquid stool, and only pass gas when I lay on my side. I think maybe I could tolerate a little fiber and some slowers, but I am cautious of getting shut down as happened before with another stricture that was resected. This is not like that yet, no vomiting or anything, just dribbling and some bloat. I will try to be a little more adventurous. I seem to tolerate V-8 ok.
