I had my j-pouch and ileostomy take down all in one procedure on November 21st.
This morning I woke with a nagging ache under the old stoma site. It is a bit tender to touch but not really uncomfortable. I don't have a fever, and it is not red. I feel excellent other than the pain. My output is markedly decreased today. I'm just wondering if it is possible to develop a hernia that does not bulge on the outside of the abdomen?
I'm not too worried as Tylenol/Advil are working (just a little bit concerned).

I think it's more likely, given what you've described, to be a partial obstruction. Try plenty of fuids, perhaps only fluids, for a while. Hopefully it will resolve on its own. If you start vomiting or the pain gets extreme then you'd need medical attention. It wouldn't be crazy to call your surgeon's office with your concern.
I concur most likely a partial instruction. That's where it happens often. Definitely increase fluid intake and decrease solids until it releases and trust me you will notice the difference. If you can move around and stay active that is the best thing to do. We are prone to lay down when we aren't feeling good but it's probably one of the worst things to do in these circumstances. Gravity is your friend right now. Good luck. I also agree if you become extremely nauseous and start throwing up you might have a total obstruction that will require medical attention. Let's hope for the best.
It may be a good idea to call your surgeons office just to let them know what's going on. If it is a partial obstruction, you'll mainly be waiting and monitoring to make sure it passes and doesn't progress. If the pain really increases, you can get pain meds to help control it. And if it does progress, you'll want to be in touch with the surgeons office (not only for the meds, which they'd probably send you to the ER) to help monitor the situation. Don't ride out the pain without calling and checking with them. Obstructions often resolve but can have serious consequences if they don't.
