Hi all,

Hope everyone is well.

A few hours ago I pooped but as I did the stool came out in an explosive manner and I immediately felt sore. The pain hasn’t gone away and is now affecting me in a way that worries me.

The pain is centred around the end of the pouch. It seems to come in waves and it feels worse when I tighten my sphincter muscle. I’m wondering is it haemorrhoids but I don’t think I ever had them before so I don’t know what they feel like.

My appetite is fine, I don’t feel nauseous and I’ve no fever. I haven’t taken a poop since  earlier but it doesn’t feel like a blockage, and the pouch is gurgling away as it normally does. I could definitely go right now but I’m worried about the pain when I try to pass the stool.

I’ve had the pouch for 25 years and I don’t recall anything like this before. Has anyone experienced anything like this?

You could have stretched the 'cuff' area- where the rectum stump is connected to your pouch. the incision area may have some scar tissue that you stretched or tore with a poop with a larger circumference than the incision area. It takes a day or two for the pain to go away.

I've been sore in that are too for a couple of days, sometimes from over straining. Later I learned it was due to scar tissue making the opening the size of a pea, very different than you but created similar pain. I take a warm epsom salt bath and/or sit on a cold pad to reduce swelling.

Eat lower fiber food that is easy to pass through your sphincter for a few days, like FODMAP. take a laxative (if they work for you) or some magnesium (works better for me).

I had a bowl of bran flakes and a fried egg sandwich (white bread) up until I went.

Stool was watery and loose as is usually the case. There was no sign of any blood. When I pooped it felt very tight, hence the explosive nature of it.
I possibly could have drank more water but it wasn’t much less than what I’d usually have.

I have talked with other J-pouchers that have pain with raisan bran, since you had bran- maybe that was related.

That's good that your drinking water though and that you stool is watery, that keeps everything easier to pass in my opinion.

I am glad your feeling better! The Calmoseptine has good properties for butt pain, maybe you can use that next time too.

I did at first and it seemed to burn. I might need to try it again. My problem is up inside and that’s why I think it’s a fissure or fistula. I don’t think I can take another surgery. Every ointment or cream I tried—just sets me on fire. It doesn’t seem to go to new doctors. They don’t seem to be able to help. It’s very discouraging. I don’t have a very exciting life. Just try to be still to stay out of pain. Walking very much is not my friend.  I used to walk 4 miles a day.  I loved it. I’m 71 and weigh 140 lindane’s 5’2. Goin on vacation with family is difficult because I can’t walk very far or I’ll start burning, spasms and have to go to the bathroom. Wish it would heal. I feel that I’ve tried everything

Yeah not everyone can take the Menthol in the Calmospetine. The Menthol has a perpperminty effect and that can cause a burning feeling in sensitive people. I have used the Calmoseptine for vaginal yeast infections as well and it cured my vaginal yeast infections- of course someone with menthol sensitivity like you, probably would not be able to handle it around the vaginal opening like I can.

Since you have a burning sensation in the inside, I would definitely suggest getting a colonoscopy ASAP! There are suppositories that can treat anal fissures in the inside of you. Even proctofoam can help as well. I would definitely have a pouchoscopy scheduled ASAP

Not a cure, but topical lidocaine gel is numbing. You can get 5% without an Rx on Amazon.

Also, topical vasodilators like nitroglycerine or diltiazem are used to treat anal fissures. Need an Rx for those. You don’t need invasive scoping to diagnose. Your doc could probably diagnose based on symptoms or a simple anoscope in the office.

I get a crazy burning pain that comes and goes about an inch inside on left side. Nobody could find the issue , Dr Shen, or my surgeon or my new GI in the past few years. On my last scope a year ago my GI said my sphincter muscle has alot of tension and that could very well be what the pain is from. Tried the fissure salves but didnt help much at all. I am having a manometry test soon to se what the muscles are doing and then i will see about Pelvic Floor therapy or Biofeedback. I think because I do have very minor random leakage since my redo in 2015 that i am subconsciously tightening all the time to try not to leak.. but if i do things to relieve that tension wont i leak more?
