So I still think I have hemmeroids and now it hurts to sit, stand up, adjust position, etc. I’ve never had internal hemmeroids. Doc just said to use cream and if it gets worse (and this lain is new) to call him. But, I’m weary about inserting anything into the rectum after the j pouch. Any ideas? Do internal ones hurt when you sit? I might just get a donut to sit on.

Hemorrhoids can be very uncomfortable. A donut to make sitting less painful is a good idea. Others can share their additional ideas for immediate relief, but I want to suggest that it’s critical to avoid straining on the toilet, to give them a chance to resolve over time. In addition to being gentle on your body while on the toilet, keeping the stool at an easy-to-empty consistency is key. That’s what makes it possible to poop without straining. This is a balance of fiber, water consumption, stuff you eat, and bowel slowers. Formed stool is not your friend right now - a soft texture sort of like pudding is easiest for many folks to clear.

Has your doctor actually taken a look? How confident are you in the diagnosis?

I havent had any hemmeroids but i've been inserting sudocrem up my rectum many a times finger deep. Its sounds gross but it relieves me of the itchiness instantly rather than applying it just over the top. I dont use the pointed plastic that comes with tubes for the same reason that I am scared to put it in my pouch. But the finger works perfectly fine.
