I would suggest that you change up your eating pattern. Some people say don't eat at night, but for me, not eating just makes my gut churn and complain with gas. The only time late eating interferes with my sleep is when I eat pizza late at night. I get up once or twice otherwise. Sometimes I sleep through.
Plus, you may be slowing your gut too much during the day, so it gets saved up for the night. I really don't know when you are taking your bowel slowers, so I am presuming...

I almost always sleep through the night, but I think if I took that many bowel slowers during the day the only time I *could* empty my pouch is at nighttime. I think that Tincture of Opium is just saving things up for later.
scott been weeding myself off that was taking 4 a day down to 1 a day not sure if i can just stop taking now

Stopping long-term opiates suddenly can be ugly. It won't kill you but it sure won't feel good. Have you ever tried the same dose of Tincture of Opium at bedtime, instead of in the morning? It might be worth the experiment. I think of Tincture of Opium as the last resort of bowel slowers. It sometimes helps when nothing else will, but 9 months in seems a bit early to have tried everything else. Do you have a diagnosis, like pouchitis or cuffitis?
I only take bowel slowers at bedtime, since (unless I'm getting on a boat) that's the only time I really want to slow things down much. I haven't struggled with some of the very high frequencies people report, though that seems to result more from a significant response to a small amount of stuff in the pouch (e.g. with pouchitis) than to things moving too quickly through the gut. Remember that no matter how much you slow the gut all the stool will get to the pouch eventually. If you're running to the bathroom frequently with little result it's important to find out what's making you run.
Why don't you think you have pouchitis? Frequent, urgent, minimally productive bathroom trips is a heck of a hint. For that matter, why were you put on Tincture of Opium right away?
I won't discourage you from quitting smoking, but (just based on the symptoms) a course of Cipro might possibly fix you up pretty quick.
Not likely. Blood is not typical of most pouchitis, and pain is more crampy than anything else, at least for me.
