Yo male 27 years old from the Netherlands here.

On 18 januari I had my surgery to get my j pouch. Before the surgery I still had an inflation in my rectum from my colitis ulcerosa. So even before the surgery I still had to go to the bathroom to lose some blood/slime once in a while.

After surgery it seemed like i could still control myself to hold my feces and not need to go the bathroom each half-hour.

But recently I have had some problems controlling myself. I do have loperamide to decrease the frequency of diarrhea. But I can't seem to clean my anus area good or have it protected.

I have used coloplast citric Barrier but after going to the bathroom 1 time the faces tend to stick to the cream. I tried using bepanthen baby creme but this also only helps with some relive but does not prevent the anus area being irritated. A family member had the same problem and he only used vaseline but this helped him tremendously. Unfortunately the vaseline dit not help me either.

Any recommendations for certain creams to prevent the anus area from being irritated?(I saw other people talk about using a bidet at home but I'm still young and want to go out once in a while)

I was also wondering if other people have problems with certain food. I noticed sour food like an orange, lemon or even sour candy will get my colon irritated.



So my 2 main questions are:

What are creams you use to prevent the anus area from being irritated?

And what kinds of food tends to irritate your colon/pouch?

I use calmoseptine or zinc ointment to protect the anus (but it does wipe off so must reapply) and aloe to help heal the anus when it is already irritated.  I also use "Forces of Nature" brand oil called "Fissure" to heal the anus when the aloe doesn't seem to be helping. It must be reapplied as well.  I make sure to keep my nails short on my pinky finger for when the irritation to the anus is slightly inside.  The bidet can help a lot when you are home.  When you go out, you can take a portable bidet with you, or just decrease the irritation when you're home and expect it to increase when you're out.

A lot of people here use psyllium (I like fine ground organic psyllium husk powder, a lot of people here like metamucil which is mainly psyllium) before/during meals.  This will soothe and coat the intestines, slow down the digestion and hopefully you won't have to use the toilet as often.  I wish someone had told me about it when I first had my j-pouch.  I didn't know about it for the first 18 years and when I found it, I was amazed!  I had always thought it was only for constipation.  You'll have to experiment with what amount works for you if you want to try it.  You can start with a small amount mixed with the recommended amount of water before or during meals.  Some people take capsules when they're out and about.  Make sure to drink plenty of water with the psyllium, especially when you first start; you are still healing from the surgery and you want to be gentle on your guts.

Many people on this forum have stated that sugar and alcohol irritate the pouch.  People have also mentioned peanuts, tomatoes, dairy, and wheat gluten are irritating.  You can find dietary discussions all over this forum.  You'll have to experiment with what works for you but I'd say that avoiding sugar/alcohol will most likely help a lot.  My guts are much happier when I avoid that stuff.

30  year j-poucher here. I tried various creams in the early days with not great results. I'm retired now so can simply rinse with home bidet and pat dry with a soft cloth I keep handy. I can manage with just wiping with toilet paper when away for the day but if I have to be away from bidet longer than that I have a squirt bottle I rinse with, then pat dry with toilet paper.  If you have the means available running water gently over the area while passing stool can help dilute the acidity and reduce likely irritation.
