I think your internist knows what he's talking about. You could try treating heartburn first, to see if it changes anything. Either antacids or something like Zantac would do the trick. 

If you're worried about the heart question, your internist could run a test or three. 

I agree. Upper GI symptoms are usually esophagus/stomach/duodenum related. Cardiac origins are less likely, but a thorough internist would not rule it out without a few screening tests. It could even be adhesion related, which would not be directly pouch related, but history of abdominal surgery related.

It possibly could be pouch related, but unlikely. I have had upper GI symptoms with serious chronic pouchitis, but I also had classic pouchitis symptoms of frequent, urgent stools.

Indeed like what you said doctor is trying first to treat heartburn and wrote me a prescription. and he said I need to see heart specialist if this did not work. I am worried if this due to heart problem, because I don't feel I have acidity in my stomach, and I have no pouchitis

The acid problem (or even ulcer) could be past the stomach, perhaps in the duodenum. In that case you'd feel pain but not heartburn. 

when I am sitting I have no pain at all. only when I make extra effort like running, fast walking , biking, stairs climbing. adhesion is new reason which I did not think about

 Long story short , I asked them to remove my gallbladder & alls been fine since..
