I downloaded a PDF of Bo Shen, MD's Chapter 27 Maintenance of a Healthy Pouch from his 2019 book on IBD from my institution's med library. If you PM me your email, I'll try to see if I can send it via that route. Tried posting it here directly but it failed. I need to learn more IT tricks!

I’ve seen this, and I like it. I would also suggest this article even more (“Clinical guidelines for the management of pouchitis”). The diagram on page 6 shows the options you have at each phase. I have discussed this article with my gastroenterologist and he appreciated that I did the research myself and had the discussion.
