Hi Guys, firstly here a little Background about me. I had a stoma from the age of 5, due to UC. Then at the age of 12 I had the reversal of my stoma, and got a J pouch. First 2 years were brilliant, and then it slowly went downhill. I'm 17 years old now, and I have experience Bedtime problems for almost 3 years. Almost every night I either leak/soil/smere etc. It's very rare that I get a clean night. I currently take 4 loperamides before bed, and I have tried every trick in the book (not eating 3 hours b4 bed, i take VSL3, going b4 bed). So anyways I raised this problem with my doctors on a number of occasions and it turned out that I had a bit of inflation. They gave me antibiotics and during the first week or so of the antibiotics, bed times were fine. However the next week in, bed times deteriorated again, and after the course my bed time troubles returned. I have been on these antibiotics twice (for 2-3 weeks) and they only seem to work for a while. During the day I am fine, I go to the toilet 5 times during the day, and I feel so great. I never have any pains, blood etc. It's just bed times frustrate me, and I'm desperate for it to be sorted.

It would be great if someone could help me out.

This far out from surgery and the lack of improvement from antibiotics would lead me to believe it is not pouchitis, but sphincter weakness. You may want to increase your Imodium (loperamide) dose, as you are taking half of the maxium dose. It is a good choice because it tends to improve sphincter tone (the sphincters tend to relax during sleep). You may want to take it morning and night, so that you have a good 24 hour coverage (there is a long half life and overlap from day to day).

Pelvic floor strengthening through kegel exercises may help too.

Another option is to add a fatty snack before bed, as this tends to slow digestion.
