Past 2 days have been straight, urgent diarrhea. Like pee diarrhea up to 12 times a day. My stomach is making loud noises.

I have abdominal pains and my stomach feels hot. No flu though. Joints hurt.

I woke up with a wet bottom and sheets. This rarely happens. I've lost 5 pounds since Sunday night when this first started happening. Loperamide isn't working. I called my GI and left a message. Don't know what it is but I don't think I can go another day like this.

This is most likely food poisoning/gastroenteritis, just like folks with colons get. For it to be a "flare" there would have to be something like cuffitis or pouchitis "flaring" (worsening). We are at greater risk for dehydration, of course, and that's your greatest risk now. If you're no longer peeing a few times a day, or if it's dark, you need to take some action. If water alone isn't doing the job, oral rehydration solution (or Pedialyte) might do much better. If it's at the point of getting out of control, IV fluids at the ER are wiser than ignoring it. The diarrhea will make you miserable, but the dehydration can do you real harm.

The underlying problem will usually resolve on its own in a few days. If you've got salmonella or giardia, though (or a couple of other infections) then it's best if it's diagnosed and treated.
