Hey guyys. So I have had my j pouch for 12 years and it was built wrong and cleveland finally figured it out. Last Wednesday I had surgery and I thought I would either have a permanent ostomy or a temporary one while the new j pouch was healing, but I woke up with no ostomy and just a one part revision surgery. Has anyone ever had one of these? What should I expect for healing? I was shocked to wake up without the ostomy. Here's hoping I get optimum use out of it. Just ate today for the first time and its achy. Any advice would help. -Joe

The problem is that revision is sort of a vague term, surgically. It could mean any number of things were done, because basically it just means that they repaired, adjusted, reduced, advanced, supported, etc. It all depends on what was wrong with your pouch.

I think the obvious answer is that whatever was done, it was either minor enough to not require a diverting ileostomy, or conditions were perfect in there to not need one (tissues in great shape, no tension on the suture lines, no steroids on board, that sort of thing).

So, unless they messed around with your sphincters, you should not have any issues with incontinence, serious anal pain, etc. I would believe them.

This does not sound like it was a full pouch reconstruction, just a repair, simple or not, so it seems appropriate to me. A lot of this stuff falls to judgement calls at the time ofthe surgery. There seldom is an "always or never" rule. This is where having experienced surgeons pays off. They know what conditions are right for omitting the ileostomy, which has its own set of risks.
