my husband is 36 years old and has a urtheral to j-pouch fistula.he has had j-pouch since he was 18 years old due to uc. he has had j pouch repair surgery. now has ileostomy again.he has suprapubic catheter. dr. tried repairing fistula with mucosal tissue,and filling fistula tract with tissue but didnt work. anyone else have this problem? any other surgical procedures that might work?

This is a rare complication, fortunately for most, unfortunate for your husband. Fistulas in general can be persistent and difficult to deal with. When simple procedures fail to close the fistula, the gracialis muscle flap procedure has been the most successful operative repair. Otherwise, a pouch revision or redo is left.

Has he seen a surgeon who has particular expertise in dealing with this type of j-pouch complication? If not, I think seeking a second opinion might be in order. I'm sorry you guys are dealing with this.

Hi, I hope your husband can find a good surgeon with the experience to repair his fistula. I have had a perineal- vaginal fistula since my pouch removal and after 2 years of numerous procedures even new ones that hadn't been tried before at the hospital I was at, I still have this problem, my pain is worse and everything is worse really. I have recently moved and I'm hoping my new surgeon who I see on 19th feb has the experience to repair. My old surgeon referred me to a general surgeon and a plastic surgeon as they said one would need to take me to bits and the other would need to put me back together again, but I'm not sure what this new surgeon will say. I'm hoping if it is too complicated for him he will pass me onto a more experienced one.

I am so sorry you and your husband are dealing with a fistula. They are difficult to deal with on so many levels.

My best advice is to find a surgeon who is an expert in fistula repair-someone who has done many of them. They are difficult to repair.

Mine was repaired at Cleveland Clinic by Dr. Remzi. Is the Florida CC location closer to you? It really is important to not have someone gain experience at your husband's expense. Too critical a procedure.
