Hey, advice  sought please! 

I think I have Anismus issues with my jpouch as I'm really struggling to empty and it feels like my pouch 'slams shut' when I try to go.

It's been going on 3 months now and I've had a defecogram and am waiting on results. I think my internal sphincter and/or puborectalis muscles have tightened or a contradicting each other....It's either that or a partial prolapse. Like I say I'm waiting on results to hopefully determine.

If it is Anismus, I've read biofeedback is the first port of call for treatment along with Botox.

but is dilation or surgery an option as well?

I have this same issue and mine is due to scar tissue at the anastomosis connection. I am due to Cleveland Clinic for some additional tests as well. I have a very had time emptying my pouch and I also have multiple fissures and anal stenosis due to this. I am not sure this is your issue or not. I have had several dilations and my anastomosis always closes back up. I am looking at some surgery to cut into the connection or a possible pouch redo if that is what Cleveland suggests. I was also going to inquire about botox due to my painful fissures from straining.  Let me know how you make out.
