I have multiple fissures and a jpouch. Doctor tells me that it's more difficult to heal the fissures because of my bowel habits. Currently using nitroglycerin ointment and taking lomotil and Imodium to try to slow things down. Does anybody with similar experiences have any suggestions? I have switched doctors because of hospital choice (my preference) and have been very fortunate in the doctor dept. Saw a surgeon 2 weeks ago and he changed my fiber supplement to konsyl and that is helping with consistency of output. I have a lot of pain from these fissures though and don't want to overdo it with the nitro. Becoming more and more depressed every day...judyk

yup, painful aren't they! my sympathy as i had a huge one. tried everything and was so difficult to heal as went on for years without adequate treatment. here are some suggestions:
1--nifedipine ointment helped only so much. started at .2% and greatly helped at 2%. i think you can go up to 2.5 or 3%--simply don't remember.
2--sitzs baths. took up to 3 or 4 a day.
3--all this to say what finally cured it was Humeria, which i took for pouchitis, although it didn't work for that it cleared up the fissure. it is expensive med and my insurance didn't cover very much, but was worth it.
so there are three suggestions you might run past your doc.

Thank you! I have been doing the sitz baths regularly. The ointment only worked "so much" because of how many times you go to the bathroom, or for some other reason?? I have an appt with doctor on Tuesday, will ask about the Humeria.
I think the Nefedapine had limited effect because I had episodically used the .2% for several years and became immune to it. When the nurse bumped me up to 2% the med was very effective, but did not completely heal. This was about 5 years into the fissure, maybe longer.
It it is possible to have surgery, but I've heard too many horrors and declined. Then the humeria cured it. That was several years ago and it hasn't returned. But expensive. I'd pay again.
Baths hs are very helpful. Good luck and I'm curious about the doc's opinion. Janet
My surgeon gave me Botox injections for my fissure in August and it has seemed to work. The injections were also very tolerable, very little pain.
