I've had my j-pouch since 2003, and literally no issues the whole time, until the last 2 weeks....

First 2-3 days - i had the most mild side stitch feeling (no pain at at, all but enough to notice something) in my lower right abdomen.  Literally nothing

Next 8 days - continuous mild cramping throughout my lower abdomen (both sides); constant stomach gurgling noises; increase in BMs from 5-7x to 9-12x's per day.

Last 3 days - Constipation seems to have set in, as my frequency is down to 3-4x's per day (and even those trips are mostly forced trips where very little comes out and i strain a bit).  And for the first time since my 2003 j-pouch surgery i've been having stomach ache feelings, as if i'm constipated. 

The kicker is, i just had my annual pouchoscopy 1 month ago w/ biopsies taken.  And the doctor said the pouch looked good w/ only mild inflamation.  I just now went back to my doctor today who said he thought i should give it a month and see how I feel - thinks it could just be a GI virus or a bacterial imbalance in my gut.  He said he would have noticed pouchitis or strictures in the pouchoscopy he performed less than 30 days ago.

His advice was take some probiotics and come back in a month if I'm still feeling the same.  I'm just a little anxious as it's the first time i've really had any issues w/ my pouch and the last week i went down the rabbit hole of googling......i self diagnosed myself with pouchitis in the first 10 days when frequency and cramps were going on, then changed my self-diagnosis to stricture once i became constipated, and now i've changed my self-diagnosis to maybe i have IBS?!

Ugh. I’m sorry you’re having these issues. I feel like pouchitis can rear it’s ugly head quickly so the fact that your pouch looked good 30 days ago isn’t saying much to me.

For me, I know I have a stricture which causes difficulty “going”, which cause gas, which causes pain and gurgling. Which can then lead to pouchitis.

My doc usually suggests Canasa suppositories. The use of them can help to “open up” the area and also reduce any inflammation.
