I've had my pouch for going on 15 years and have lived pretty comfortably with the various "issues" we j-pouchers experience: frequent trips to the bathroom, itchy butt, anal strictures, and low-grade chronic but manageable pouchits. I am on 500mg of Cipro twice a day and use hyosciamine to control spasms.

  Recently I have been experiencing bouts of minor incontinence during the day. (I think being vertical contributes to it.)  When I get a spasm or urge to go, sometimes a little stool drips out that I can't control.  It's always a small amount for which a panti-liner is sufficient, but it requires that I quickly find a bathroom to change the liner. It has been getting more frequent which worries me that my anus is "wearing out" (weird, I know) and that eventually I will lose more control.

  Has anyone else experienced this with a longstanding pouch?  Any suggestions, medical, dietary or otherwise? I am 60 years old and in very good physical shape. Thanks!

I found that after a few years of Cipro alone it stopped being as effective. I tried some others (Xifaxan, Augmentin), but ended up adding Flagyl and getting good results with both Flagyl & Cipro. I did keep reducing the doses until I found the minimum effective dose. In case it helps, I found once daily to work as well as twice daily (of the same total amount per day).

 

You may have something else going on, but if it's slowly returning pouchitis symptoms then a treatment change is probably necessary.

Yes......after 12 years......except, I'm experiencing periodic "gushers"!  Colorectal surgeon says it is related to foods/drinks.  Told me to add Citrucel to my diet.  I notice a connection between caffeinated coffee and these symptoms.  Need to try switching to decaf.  Not ready to give up coffee altogether!  Complacency is not for us J-pouchers, I guess!
