No matter what I do, I always seem to have some type of butt rash. It’ll go away and then in 2 days it’ll be right back. I have a bidet and am going on year 8 with my pouch. I used to use ilex but that’s hard to find now. I try to use my daughter’s a Triple Paste and that helps but…it comes right back. Yesterday it was so bad there was blood on the toilet paper bc I wiped so much. Idk what else to do. Open to suggestions!

I’m surprised that you’re wiping so much if you use a bidet. I do blot to dry with TP after using the bidet, but do just about no wiping.
Does the rash appear even if you’re religiously applying the cream? When do you apply it? When you say “it comes right back,” is that after you stop using the cream, or is it even though you’re using it?

I am an Ilex user as well.. They have emailed me a few times and stated product will be back in stock beginning of 2024.. Stated several times they are not going out of business as some rumors are out there.. But where theres smoke theres fire so who knows... I havent found anything close to Ilex at all
I use prep h cooling gel (Walgreens generic) for the peri anal area.
i use Desitin (35% zinc) for the diaper rash on the inner cheeks.
both work very well for me and the diaper rash usually clears up overnight.
Scott - Well I don't have a bidet at work and that toilet paper is so harsh (I work at a university). I guess I could bring wipes but it's hard to get those to the bathroom without anyone seeing. And the noise of opening a pack of wipes up in the bathroom - it's just embarrassing with my students and coworkers in there. There is a handicap bathroom that I could use but it's usually in use (with kids going number 2).
The rash appears by the end of the day. Usually I don't apply cream during the day, just in the evening. But then, it's past the point of being manageable. I'm good to go in the morning (the next day) and then the same cycle occurs - I go to work and it gets bad by evening.
Sometimes when it's really hot and I'm working outside and sweat a lot, it gets bad too.
Thanks News577- I'll look into that. I've tried Ilex Calmoseptine, Triple Paste (I feel it works better than Desitin for me), Coloplast Barrier Ointment. Ilex is the best barrier cream, but triple paste heals it....so I feel like I have to choose. Sometimes it's so bad it hurts to sit. Just so raw.
I get you Pouchomarx! I'm going to buy a big tub for both the bathrooms when it comes back in stock. Wonder what the hold up is?

Their email stated it was a distribution issue, stated two of their key ingredients are on backorder.
Using the toilet paper partly wet makes it less aggressive. You could drop a little bottle with water or aloe gel into your pocket or simply spit on the toilet paper.
@Bubbapup, I take a half roll of decent toilet paper with me in my bag when I go out of the house.
Maybe you could get a smallish sporty-looking cross shoulder bag to keep at work. Something that looks like it has a tablet in it maybe. Should be inconspicuous to take to the loo.
Also, have you heard of the spotlight effect? It’s the tendency of the human brain to overestimate how much attention other people are paying to us.

Those are good suggestions, thanks. I’ve been putting some wipes in a zip lock and put them in a small purse and bringing them with me. It’s so tough bc we have a 5 stall bathroom that we share with students and when classes are swapping, it’s just crammed in there. I’m all loud, etc.
I agree, my colleagues prob don’t even notice so why should I concern myself with them. But since I’m obsessed with the bathroom, I DO notice when they go, if they stay for like 20 min, etc. lol. I guess I have to just get over that.

Just some other thoughts: If you feel that your stool is aggressive, you may try to watch your food in terms of acid-base metabolism (little alcohol, sausage, meat ...).
I found it most important not to delay toilet trips. For some weeks now I go there when I feel my pouch is (almost) full, and it has a clearly calming effect on my bowels. Letting it accumulate into the terminal ileum creates pressure and promotes bacteria growth. Both together means increased inflammation, if you are reacting 'allergic' to certain of those bacteria.
I thought we were supposed to hold bowel movements as much as possible to grow the pouch size…
Dan that’s in the beginning but some of us are 10-20 years out.

“As much as possible” is too strong an idea, IMO. When the pouch is new is certainly needs *gentle* training to become a useful reservoir. Once you have the capacity to hold a reasonable amount of stool for a reasonable amount of time then you should probably do whatever serves you best. Some days I delay bathroom trips quite a bit, for my convenience, and some days I choose more bathroom trips, again for my convenience. I think Steve may be taking the backwash idea too far, but I agree that there seems to be no need for a mature, well functioning pouch to be stretched.
