Hello my friends,

I am writing today to kinda get a feeling for how I should go forward.

Bottom line, I am now being treated with Cipro for my second bout of pouchitis in the last six weeks.

just as a background, I suffer from the following;

1. Irritable pouch syndrome, which I treat with levsin as needed.

2. Cuffitis which I treat with hydrocortisone suppositories for flareups, and Canasa for maintenance.

3. SIBO which I treat with Bactrim every 4 to 6 months.

And now the pouchitis has entered the picture with my first case of it in January and now my second case of it this week.

I am concerned that the pouchitis is going to become chronic and I’m going to become dependent on Cipro, which works perfectly to knock it out of my body.

Does anybody have any words of wisdom as to going forward as I work with my G.I. specialist to determine if maybe at this point in time, a biologic may be necessary to alleviate some of these ongoing issues so they don’t return on a regular basis.

I also want to add that  I have cuffitis in addition  to the pouchitis at the same time.

If it turns out to be chronic, antibiotics work just fine for most folks. It’s a bit premature to consider your pouchitis chronic, though - the January case might just not have been fully eradicated. I’ve been on Cipro since about 2008, and I honestly don’t “suffer from” pouchitis. I have well-managed pouchitis. It’s often considered best practice to rotate between several antibiotics, switching every few weeks, but that only works if you find multiple antibiotics that do the trick. If your GI thinks that a biologic might control your cuffitis as well as your pouchitis that could be a reason to go directly to a biologic.
