The big concern with conceiving after j pouch surgery is that the Fallopian tubes could be blocked by scar tissue. There is a test that can check to see if the tubes are open, called an HSG, which is done by shooting dye into them during an X-ray. Sorry if I am telling you something you already know.

I talked to my OBGYN about it around the time I got married and she said because of my history, she would be happy to order the HSG for me even before we tried to conceive so we wouldn't waste any time if I had two blocked tubes. I'm sure not all OBGYNs feel that way but if you are ready for it, it's worth asking.

Other than that, the standard medical advice is if you have a pre-existing condition that can affect fertility, you can see a doctor after 6 months of trying.

Fwiw, I conceived my first while trying not to get pregnant, and now it's been over 12 months trying to have a second.

Also, I know this isn't exactly what you were asking, but to my knowledge it's standard to have a full fertility work up for both partners before IVF, including sperm analysis, so they aren't missing something that would make the IVF cycle fail. So you would definitely do it then. But I think you meant at what point of trying naturally should you have your husband/partner tested. The standard advice is after 1year of trying,  or earlier if there are other concerns. But that is best to ask your doctor.

It took us 10 months to conceive naturally. Based on OB's advice, around 6 months of trying with no luck my husband was tested and I had an HGN test to make sure my tubes weren't blocked from scar tissue. Knowing that everything was working and it would hopefully be possible to naturally conceive allowed me to stress a little less about it. I would recommend having an HGN done if doc will do one- good to have the power of information.
