I posted weeks ago about going on Budesonide/Entocort for cuffitis. I waited couple weeks before going on it, while continuing Cortifoam rectally, Canasa, and 10 mg Prednisone. Tried to change up from carbs I rely in for bulking up output to anti- inflammatory diet. Granted, not enough time for diet to help, as bleeding, frequency, leakage, and horrible butt burn has increased. How long will it take for Entocort to help? It's been 5 days at maximum dose and no improvement. I am off the 10 mg.
prednisone but continuing Cortifoam. At this point I am concerned that I may have anal fissures or even pouchitis. Not sure what else Dr. could prescribe to help me get better..... Cipro gave me incontinence, Flagyl some numbness. If it is just cuffitis, as was originally dx from scope, is Entocort going to work, or am I too far gone? Thanks in advance for your input! Laurie

I experience this often and I do also have anal fissures w cuffitis. Augmentin helps me tremendously with both of these issues. When I try to eliminate it completely I am one suck puppy, but I am thick and many times desperate for a rest from chronic antibiotics. When I going through this period of no antibiotics, I use rectiv cream( very expensive and does give you a bad headache initially), but it helps with the burning anal pain from the fissures.

Rectal meds never seem to help with my cuffitis as mine is chronic, but augmentin keeps it relatively asymptomatic. I tried enticory for a bit with no real difference and was on cipro for a long time. Also, cortifoam always further irritates my cuffitis. I have some luck w anucort and Canasa but when cuffitis is active I can never retain them as long as needed to have a real impact. Hope this helps.
Thanks Jeanne and JJA for your input. Will talk with nurse tomorrow about possibility of Rectiv and see if Anucort might be better choice than the hydrocortisone foam I'm using. I had irritation with Canasa and have wondered if Cortifoam is irritating too. I would have expected improvement once on Entocort and am wondering if they'll have me use a a kick start dose of Prednisone since Entocort not working. Just so frustrating to have these surgeries because they didn't want me to stay long term on Prednisone, and now seems that I may be on steroids anyway. Assuming Rectiv is a prescription- looking at it online seems like good possibility. Also assuming they can dx anal fissures with office exam. Since they won't want to scope again, they may have to guess on
Pouchitis, or just treat with antibiotic. Need antibiotic that won't give me C-Diff! Had several times with UC, and am horrified to go down that road again, esp. because never know when it will hit. Thanks much!

Amal fissures are easily diagnosed by symptoms and anal exam in office. Rectiv is by prescription. It costs me about 160 w insurance coverage. It is pricey and be aware of the very quick, strong headache you will get after using. This does diminish after using it for a bit, but the headache can be very severe initially.
