I'd guess hemorrhoid as the most likely cause.

You don't mention if this bulge is painful. Could be fissure or hemorrhoid, or even abscess.

Jan yes it is painful when I cough yell sneeze was going to post pic of it but not sure I can wife took pic so I can see

Most likely a fissure or abscess then. Hot sitz baths 4 or more times a day until you can be seen to get this sorted out.

Sounds like an abscess. You'll need to get it drained sooner then later. It will only get more painful.

So the last 2 nights woke up at midnight and up 3-4 hours going every 30-45 min and only tiny amount of still comes out do I have pouchitis?

Could be, but with your other signs and symptoms, if you have a fissure or anal abscess, it could lead to frequency and urgency with incomplete emptying.
