I had one prior to my j-pouch and it is now a source of much suffering.   Before I learned to manage it I was brought to tears more than once.  But my surgeon said trying to remove it would mean terrible pain and she recommended trying to live with it. 

The best thing for me is to keep the area very clean and dry and avoid wiping as much as possible.  Also apply a barrier every time you wash, but make sure you are dry first.  Some people here swear by bidet seats, but I'm still making do with a squirt bottle of warm water.    I also find sitz baths in warm water helpful. Then blot dry instead of wiping.  You can dry with a blow dryer on a cooler setting.  Apply a barrier every time you wash, but make sure you are dry first.

When it gets really bad I use cortisone cream for a couple of days until it calms down again.

I suffered from hemorrhoids soon after takedown. After a year of suffering, I finally got them under control. Ilex paste is great to alleviate itching and works better for me than any hemorrhoid specific ointment. There are special bath salts by Better Bath, Better Body that are meant to alleviate hemorrhoid discomfort. I just put some in my bath water. The third thing that helps is to use a barrier cream every time you empty your pouch. I use wipes to make sure I'm extra clean.
