I had an intestinal perforeration 3 months ago and had to do a resection. It was well above my pouch. Last month, after takedown surgery, I developed a fistula right at the surgery site. I was put on TPN to let the fistula heal. Just as it healed I developed an infection in my blood, possibly from the TPN. The PICC line was removed and I abruptly stopped TPN. Has anyone been in this situation? Currently there is some pain as I digest foods. Any suggestions for getting back on a regular diet?

Hello, Mad. I would treat your insides and your pouch as if it were a 1 year old toddler for now. Highly nutritious, soft foods for a toddler. Try things like soup (cold soup would taste great in the summer heat). Fruit smoothies for vitamins, add Greek yoghurt for protein, or avocado for good fat. Tofu or eggs. Fish, chicken, and peeled, well cooked vegetables. I keep bags of frozen fruit in my freezer for smoothies so I always have them handy and I don't worry about the fruit going bad. Break down your food as much as possible before it gets to your intestines and pouch. If the food is hard or cooked dry, don't eat it. Choose high protein foods so your injured tissues can heal. Avoid hard nuts and seeds, and greasy foods. I hope you feel better. Your experience sounds rough. Best wishes.
WinterBerry gave some great advice above. Use real foods, cooked, and try and use a blender or food processor on foods like chicken, roast beef, etc. Soft broiled salmon or halibut has protein and such. Also, make sure you get enough fluids.. this is not an advertisement, but in the past, two products that helped me were a high calorie high protein elemental liquid food, known as Peptamen..vanilla flavored, drank ice cold, was the best...Also, a product I still use often, Ceralyte 90 natural, it is an oral electrolyte fluid..the ninety strength has great mixture of electrolytes that make it easy to absorb the fluid when you drink . It is a powder that you mix with about 1 L of clear fresh water. It is best served very cold also. Oh, the infection you had may have been what is known as a central line infection. Also, is the doctor aware or did he or she order that the TPN be discontinued, perhaps they can also offer advice on nutrition and helping to heal your pouch and the fistula. I myself am currently on 3 L of TPN per day via a central line.
