My doctor asked me the same question, having a pouch for 34yrs with all my issues would I ever consider going back. 

My reaction was shock, I have worked so hard to be "normal" even with all my issues and horrible meds that would be the thought for me.

Before you call it quits on your j pouch, I recommend that you get a second opinion from a doctor who has lots of experience with problematic pouches. Cleveland Clinic has an excellent reputation for dealing with difficult j pouch problems. If the decision is to have your pouch removed, you may want to consider the k pouch or closely related BCIR, both of which avoid the need for an external bag and have a high rating of patient satisfaction. I had severe anal irritation coupled with high grade dysplasia and opted for a BCIR which worked out well for me and provided a very good quality of life.
