I have been diagnosed with something that is likely an anal fistula about 1cm inside. 4 different doctors gave me 3 possibilities for what it is (fistula, thrombosed vein, skin tag),  but it is most likely a fistula. I keep having flashes of burning every so often out of nowhere, and general pain/burning every time I have to go, which is about 8 times a day.

I have tried Calmoseptine, 2 different prescription lidocaine creams, and Sitz baths, and nothing is stopping the pain. It's been going on for weeks.

Is there anything other than surgery that can stop the pain? Something that could just kill the nerves and just turn it off?
