I have an appt in June, but also did   NOT receive paperwork either. Every time I call, I leave a voicemail. The very first time I called I actually talked to someone named Linda. She said she would send paperwork and hotel recommendations. I did have CC send my records to NY. (Another process—their fax machine was busy forever—finally just mailed the form)

I will keep trying!

How odd...I've spoken to someone three times. The first time they scheduled the clinic and scope appts no problem, but when I didn't get the paperwork I called back to confirm and they had lost the scope appt. So I'm a little uneasy about this, but it's somewhat reassuring to hear that others have had the same thing happen.
