I recently had a scope and they discovered pouchitis and what may or may not be Crohn's as the ulcers were high up and put me on tapered prednisone for 6 weeks, I started it on Tuesday . My butt issues are so severe now with something vicious that is making it impossible for me to walk and I walk a lot for my job. They gave me a cream for a fungal infection (used for 6 days) and it did not work. I use Nefedipine as I believe there could be a fissure there. I use Lidocaine and take sitz baths regularly. Am i missing something ? Extremely sore to the touch on the sides, excruciating at the anus when wiped (this area has very visible red slits which look like cuts) I am at my wits end with this and cannot even play with my kids.. Any help or advice would be appreciated.

Ilex paste should work with severely excoriated perianal skin. https://www.ilexhealthproducts.com/usd/
Antibiotics are usually prescribed for pouchitis before prednisone. Have you tried and failed them?

Yes, I had a course of cipro which did not work.
i am going to try diltiazem tonight on anal area and keep sides dry and may try again with fungal cream.
How about Flagyl, tindamax or Rifaximin? Cipro is not the only pouchitis antibiotic.
If the diltiazem does not work, please try adding Ilex to protect.

Jan,
Thank you for the advise, I was using every cream in the world to get rid of the fungal infection and all the nefedipine in the world for the fissure.
I should have just used Ilex from the start and kept the area dry and clean with plenty of sitz baths.
Hang in there - that butt pain is awful. Hope the ilex does the trick for you - Jan is so great - she is the angel nurse on here. I hope she knows how much she helps everyone on this forum. Feel better soon and know people out here care.
