Osama, has your gastroenterologist offered any ideas or tests? Do you have abdominal pain during these episodes? Are they reliably showing up every three months, or is it more like "about 4 times per year?" How long do the episodes last? Have you ever tried Cipro or Flagyl for them? Have you been tested for C diff, or other persistent parasites like Giardia or amoebas?

This is a bit frequent for a basic GI infection, like norovirus, but if you are routinely eating food from a contaminated environment you could get reinfected frequently. Is your favorite restaurant a little too casual about cleanliness?

i take Cipro and Flagyl every  time i have the bout , its take usually 2 to 3 days to get out of it , but couple of time it took one week and two weeks , the blood test at hospital when i have the bout shows that i have inflammation , but after one day the blood test is normal. i tried all kind of diet , i don't eat meat , i try to eat cooked food when i can.

Mady,

I have a different idea and possibly off-the-wall but worth checking into...Your hormones.

Not all periods are created equally and your hormones do not just fluctuate monthly but seasonally too. And they have a very strong influence on your pouch.

The every 3 months leads me to believe that this could be the case. Does the problem occur during either ovulation or your periods? Is there a correlation?

Are you suffering from lower back pain at the same time?

I would get 3-4 bad periods/yr (nothing like what you describe but bad enough)...didn't link it to anything specific because in my case (also self-employed) I just powered through it. I was told that our hormones have seasons. High and low, strong and weak...so maybe in your case there is a connection...some hints that I was in a bad one were massive headaches (I am not prone to them), dizzy spells, cystitis, feeling like I was hit with a baseball bat in the kidneys, my pouch running like water, horrible pouchitis-like cramping etc...it might be worth consulting your GYN about this...you never know.
