Anal spasms, that comes and goes and pressure that just dosent go away in the rectal area. Loud stomach sounds. Liquid output, or almost constipated. Having a hard time emptying the pouch, very little comes out.

I would definitely contact your gastro and make an appointment, only he can truly assess what is really going on, please do not be afraid to go in the office if you need to, I would hate to see things progress into something worse.
Keep us updated, sorry you are going through this!
Sounds like it could be pouchitis. I have had all those symptoms many times and that is what my doctor said it was. Pouchitis has many faces. Prayers that you will feel better soon. I got my pouch October of 2008. Following my surgery I suffered with pouchitis constantly. My regular Gastro Doctor in NC at the time sent me to his mentor, Dr. Bo Shen at the Cleveland Clinic in Ohio. Best thing I ever did! He gave me some dietary advice, the number one thing was no sugar. Refined sugar that is. He also told me I needed to exercise. I have been following his advice for over 10 years and I am doing better. I still get pouchitis at times but not constantly.

I hear nothing but great things about Dr. Bo Shen, he gives great advice on pouches
Sounds very much like my experiences over 30 years with occasional pouchitis. See your MD, what have you got to lose when you have everything to gain?

I would not recommend holding it in, you should go whenever you need to; there are a lot of risks associated with holding poop in. I know its convenient to hold it in but its not safe.
Glad you are doing better though
I don't hold if i get some urge to go i go but when I'm out i know i can hold it if i need to
