4 days of joint/back pain generally is not enough to be sent to a rheumatologist. Several months of unrelenting back pain is. This could just be a virus. But, the psoriatic symptoms may be a factor. Also, if you have had joint/back symptoms in the past that you do not mention. I'd at least check in with your primary if the fever does not resolve within a week. It is low grade, so not to much of a worry.

When I was diagnosed, my back pain was predominately sacroiliac (upper buttock). It radiated to my hips. I couldn't even roll over in bed. It didn't matter what I did, it persisted. When my rheumy conducted my history, she said the fact that I had decades of various tedinopathies (particularly Achilles and plantar fasciitis) and UC, pointed to enteropathic arthritis. All my blood studies (RA, HLA-B27, ANA, etc. were all negative).

If your pain does not resolve, definitely seek a rheumatology consult.
