It does discuss corticosteroids as a treatment options for antibiotic refractory pouchitis. 

One of the problems is that there are many types of pouchitis with varied causes. Is yoursCrohn's of the pouch or another form of IBD? Ischemic pouchitis? Food sensitivities? Structural issues? Some or all of the above?

If high dose prednisone is having no effect after a week, you probably should abandon it and try something else.

Maybe another opinion with someone with a lot of experience with problematic pouches will help you get treatment that is more targeted to your specific case.

what abx failed? have you been on them frequently in the past?

what about budesonide which is thought to release in the gut more specifically.

many of those articles that Jan and others have referenced regarding pouchitis were incredibly educational. I highly recommend investing the time reading them.

I've read that article. 

I was on Flagly+Cipro (helped for the first time), than switched to Rifaxim. I also tried levoquin one. Nothing really helped. 

I got some Pepto Bismol imported, which doesnt seem to help either. 

No my doctor has put me on Decortin  and some budenoside in addition. I do not think, this is really helping. 

Trying it with 2g Rifaxim + 1g Cipro daily now. Starting tomorrow

Good luck! I usually have better results with a combination, but my GI is not keen on me doing that long term. He worries about bacterial resistance.
