C- Section Incision

Hi there,

Looking for some opinions and advice for women that have had a C-Section after a J pouch.
I had a Reverse Ileostomy with a J pouch done in 2009 for my UC and have had almost zero issues since the surgery. I did struggle with infertility but am now 35 weeks pregnant with the help of IVF. I’ve had a healthy pregnancy so far without any flare ups which I was nervous about before conceiving!

I am having a planned C-Section which was recommended by my GI and OB which I am also most comfortable with for delivery. My OB has said it’s my decision to use the existing vertical incision or the most common horizontal transverse incision. She said recovery is a bit longer/harder with using existing vertical incision but since I’ve already had surgery I know the healing process and I have to have an incision either way. I’m on the fence as it’s obviously not as common to do a vertical C-Section now days but I would like to stay with two scars instead of adding a third to my abdomen. Looking for stories of other woman who have been in this situation and what they did!

My dr didn’t ask, he just told me they were going to go over the same vertical scar. He did say why make another scar. Afterwards he said he did have to cut away a lot of scar tissue, but I didn’t have any problems healing or pouch issues after delivery. I personally didn’t need any pain killers, just some regular Tylenol to take the edge off for the first couple of days. I was considered a high risk pregnancy and had some of the best OB drs in Chicago. Everything went smooth and I healed fine. Wishing you a smooth delivery and recovery!

I didn't end up needing a c-section with any of my pregnancies, but my OBs did talk about which incision they would use if the need arose. They gave me the same advice you got (with healing being harder for vertical, but could be nice to avoid having an additional scar) and left it up to me. My decision was to get the bikini cut-- my abdomen is a chopping block anyway, and I wanted the "easiest" recovery possible. The bikini scar is very low, so there's never an instance where it would be visible. Just my personal preference!
