Dr. Don Schiller has announced that after 44 years in practice he is retiring.  After Palms of Pasadena Hospital in Florida shut down their BCIR program in 2019, Dr. Schiller in California was the only surgeon actively performing BCIR surgery in the US.  A number of Dr. Rehnke’s patients turned to Dr. Schiller when complications they were experiencing with their pouches required surgery while others were treated by K pouch surgeons.  Dr. P.J. Benyamini, Dr. Schiller’s associate, has decided to pursue interests other than BCIR.

The continent ileostomy still remains a very good option for people with failed J pouches, problematic end ileostomies and others who do not want to have a conventional ileostomy with its associated issues.  Although the K pouch is constructed in a slightly different way than a BCIR, it functions exactly the same,  has comparable success rates and user satisfaction rates as the BCIR.  Those who are considering getting a continent ileostomy should have no reservation about contacting a K pouch surgeon.  A list of doctors who are familiar with continent ileostomies is viewable on the Quality Life Association web site (www.qla-ostomy.org) under the Medical Info tab.

This is interesting Bill. I wonder why BCIR has diminished interest after all these years. I have to wonder if it is about the complexity/exclusiveness of it. I know there had been some debate about why it was essentially marketed as a proprietary procedure. But, they traveled the country teaching the procedure, so it really was not exclusive. Maybe there just was not enough interest to keep it going. Maybe the k-pouch has adopted some of what the BCIR different.

Still, it is always sad when options go away. Especially sad is that patients will not have the same follow up. Thankfully, there are many surgeons who do continent ileostomies.

This is unfortunate given Dr. Schiller's dedication. I had heard that Dr. Schiller and Dr. Rehnke had been sending several patients with complicated problems and those needing continent ileostomy to the Continent Ileostomy Center at Columbia University Medical Center, NY. Dr. Ravi Kiran is very skilled with repairing the BCIR and has been doing a high volume of these continent ileostomy procedures. Dr. Bo Shen (GI) and Dr. James Church (Surgeon) who also have expertise in medical and surgical care of the continent ileostomy have also joined the Center at Columbia.

Dr Schiller has repaired the valve on my BCIR twice now. Once in 2018 and again in 2019. It now needs to be replaced. Dr Schiller had me scheduled for May 21st 2021, two weeks away and he canceled my surgery and announced his retirement. I’m glad to hear there is still a Dr left that can help with my situation. I had no issues for 25 years but now can’t seem to get this thing to work for more than a year. I hate to have to travel all the way to NY but at least I have that option. Thank you for posting.

What made your pouch leak all of those times?

My valve after each repair developed scar tissue causing the mouth of the valve to fish mouth allowing fluid to escape. Very unfortunate. Dr Schiller concluded that replacing the valve with new tissue will solve this from happening again.

Wow that is crazy! I am sorry to hear that! I see leaks are pretty common in all types of pouches.
