I had continence problems during the first five weeks after my takedown (I had a two step surgery.) I never had to deal with it before because I don't have UC. Basically, I would have great urgency just to go a little bit, and I'd have two to three accidents a day during that time (mostly in bed at night - but the two "best" ones were in the airport and in my car driving -- Some of the looks I got were priceless.) With the B-Sure pads, Depends, and those bed pads, I was able to control the damage pretty efficiently. Not trying to scare you, just giving you an accurate picture of how it was for me.
3.5 weeks after take down, I went on a strict regimen of 1 tbsp of sugar-free metamucil in 4 oz. water after my first and last meal of the day with no water for an hour after. That eliminated the incontinence, but I was still waking up like 6 times a night to go. So a week later, I did 1 immodium tablet right before bed crushed up and then mixed with a tablespoon of applesauce. Apparently, if you crush it up and take it, the pill absorbs faster my RN told me. That took me down to 8 times a day and maybe 2-3 times a night. Now, at about 6 months out, I go about 5-6 times a day and 0-1 times a night ... if I don't drink alcohol late at night :-). For six months out, I think this is in the good to normal-ish range.
In hind sight, I would have asked my RNs to start metamucil earlier. They told me 2-4 weeks was the right time, and I wanted to err on the safe side at 4 weeks because I have a history of small bowel obstructions. I'm still taking immodium generally, but I'm experimenting with not taking it when my stool is already solid. I find sometimes I have "incomplete emptying" in the morning and will go twice in like a 1 hour span. So, I'm trying to selectively drop immodium to control that. It's my own personal theory, so there's no medical knowledge backing that up.
Also, during my brief spat with incontinence, I didn't eat anything after 6pm usually (I go to bed at 10pm.) It didn't stop the accidents. Who knows though, maybe I would have had more accidents if I had ate past 6pm routinely. I still try not to eat after 7PM, but it's hit or miss. I exercise a decent amount and sometimes I'll eat at 7:30 on those days, and sleep all the way through the night. Sometimes I eat my last meal at 6 and randomly wake up twice. I haven't noticed a huge difference.
I think the trick is just to think of the pouch adjustment as temporary until it gets better, which will take a little time. Also, everyone's adjustment is different, and I'm sure many people weren't having three accidents a day. Now I can laugh at some of the classic j-pouch moments with my wife over beers at our favorite restaurant -- It gets better. Good luck and hope your transition goes well.
