I'm 1 year post takedown & have had 1 course of Cipro in May & just completed a course of Rifaxamin both of which worked well. However within 7 days of finishing the usual symptoms return, I believe I have SIBO. 

I was wondering if taking Cipro or Rifaxamin for say 3 days would be advisable? I've always found they work with immediate effect but GI says finish the course although he doesn't have a huge amount of experience in this field. I find Cip gives me joint ache & Diarhoea, Rif insomnia & back ache hence my reluctance to keep taking it. I take Immodium & Plantago also.

I'd advise against a three-day course, even though the side effects are uncomfortable. Perhaps a *longer* course will keep the symptoms from returning so quickly. Also, you might consider VSL #3 to keep the pouchitis from returning - it worked for me for a few years. Maybe you'd tolerate Flagyl better than Cipro or Xifaxan?

 

Why are you thinking SIBO? Pouchitis returning immediately after stopping antibiotics seems to me more suggestive of antibiotiic-dependent pouchitis than SIBO.

Thanks Scott, I'm thinking SIBO only because my symptoms are mild, there is no fever, aches, sickness, just increased gas, urgency & diarhoea which I contain with Immodium & Plantago rotation. I'm probably being a bit impatient, a year is still not that long. My BMs were 8-10 before ABs, 6-8 after but then they creep up. Maybe I just need to more careful with diet, it's just so frustrating having been ill for so long you just want to be more normal. Patience is the key I guess. Unfortunately VSL3 is not available in Europe at present due to some "breakdown", the other probiotics available in Spain are no way near the strength of VSL3. I don't know why but Lomotil has the same "breakdown" here too.
