Hi 

I've had UC for the past 14 years and my last colonoscopy show low grade dysplasia in one of my polyps.

he did say that surgery is the best option but I've decided to go down the monitoring route with Azathioprine and yearly colonoscopy. 

I asked if all fails can I have a Jpouch and he said No as I have Anal Stenosis and the last 20cm of my colon from the rectum up is inflammation and it's all got to go including the rectum

The surgeon said I could but you would find the Pouch would suddenly drain 

Any advice as I would have liked this option if all fails

You may want to get a second opinion about having a J pouch, but the presence of low grade dysplasia with US puts you at risk if you do not have surgery.  Two surgical options that do not require having an external bag are the K pouch and the BCIR.  The BCIR has been approved by NIH in the UK and is now being performed by Dr. Ed Westcott in London.
