oh yes.. I had that.. when my jpouch was disconnected. It is actually called "diversion pouchitis". It is caused because the "nutrients" are not going through the pouch to keep things in line.

Generally treatment for it is "short chain fatty acid" (SCFA) enemas. If that doesn't work, then Rowasa enemas. I have never ever heard of flagyl being giving for that. The concept doesn't make sense to me. FYI... when I had it I was under the care of the best of the best jpouch GI doctors.

for me the SCFA enemas didn't work. Rowasa did initially, but quit working after about 6months. Also I had an enormous amount of abdominal pain with this diversion colitis.

For many, myself included, this is why the jpouch needs completely removing.

I had diversion pouchitis too before the jpouch was ever connected but it was nothing like what it was after the jpouch got disconnected. That was horrid and extremely painful.

kenny.. I had to get the SCFA enemas from a "compounding pharmacy." I have the recipe somewhere let me know if you need it. And Rowasa in the US is just standard ulcerative colitis mesalamine enemas. So I imagine that is readily available in UK, maybe under a different name though.

There is only one cure for this and it is J pouch excision. I tried several of the suggested treatments and they didnt work, try them maybe you will get lucky. The excretions kept getting worse and worse, mine had little smell and usually just a light tinge of red but it was obnoxious because they leaked out. They started off as thicker but got more and more watery. I had some of this leakage with my loop but with the disconnected J pouch it was much worse. I figured at some point I would have to get the disconnected J pouch removed, but was hoping to go for while before a big surgery, I only waited 6 months. In hind sight it is usually better just to get it over with and excise the pouch once one gives up on the pouch, though there are some people that dont seem to have this problem.
