Yes this could be pouchitis, but it could also be several other things; stricture also comes to mind, as does a partial blockage. Pouchitis doesn't present the same way for everyone. If you're never had it before, it's best to see your GI and have it confirmed. Your GI would also be able to confirm or rule out stricture, blockage, cuffitis, etc.

Some things you may want to look out for, in addition to the above; telltale signs you may have pouchitis usually include increased frequency, very watery stool, feeling like you are not completely emptying, and a general feeling of malaise/fatigue.
I have a K-pouch. I am lucky and rarely get pouchitis. The main clue for me that I am getting sick is when I intubate, it shoots out water-thin stuff very fast.

( I have no bleeding or fever ) Then, I go on Flagyl, 3 x per day for 12 days or so. Clears it right up.

I am a big believer in probiotics. Culturelle and DanActive works for me.
My doctor was really pushing me to take probiotics too. I can't stand the taste of VSL mixed in anything. He said I could try OTC probiotics. If they worked "great", if not than try another.

I am now going to try a 9-week course of Align ($45 at Costco). One capsule a day. I prefer capsules over tablets as I have always had a trouble swallowing pills.

Started taking them the day after I finished a 7-day course of Flagyl. Pouchitis symptoms started to slowly appear after the Flagyl, but nothing severe (Periodic low-grade fever and 1 night-time leakage, and my stools are not too watery, light in color or increased frequency).

After a week of Align, my only symptom is the periodic low-grade fever (99 temp).
